Responsibilities:

Available for new client phone intake
Scheduling and completing in home living room visits
Creates and revises Care Plan for clients via personal visits to assess patient requirements for care.
Conducts on-going assessment of patients
Records all clinical and progress notes and enters them into the patient’s permanent record files
Responsible for overseeing any medical issues as it relates to clients and staff.
Education/Discipline of caregivers
Monitors and Reviews Hurricane Plans
Conduct Quarterly Quality and Assurance assessments.
Communicates with clients and families
Speak with Staff/families during client emergencies
Available for State Surveys
