Devoted Hearts Hospice Corp and Pacific Care Group LLC are looking to expand our services in Hemet, CA & surrounding cities . We are taking applications for dedicated and reliable RNs, LVNs, Health Aides, and Social Workers to provide quality care to our patients.

Responsibilities:

• Provide routine individualized care to the elderly, convalescents, or persons with disabilities
• Administer medications and treatments according to a detailed plan of care
• Monitor client’s condition, including efficacy of treatment and medications
• Provide personal care, such as bathing and dressing patients (CHHA)
• Provide specialized healthcare such as administering medication and changing bandages
• Teach family members ways to care for patient
• Document accurately and submit timely nursing notes, according to agency standards
• Help patients understand their illness or condition and provide them with information about the resources available to them to cope with the emotional, financial, and social needs that arise with a diagnoses.

Qualifications:

• Experience in hospice, home health, or other related fields
• Familiar with Data Soft Logic and HospiceMD
• Positive and patient demeanor
• Ability to build rapport with patients
• Ability to handle physical workload

Excellent written and verbal communication skills; also looking for Spanish speakers
