Direct Support Professional

Location: Forsyth, GA

Shift 1: 8 AM - 8 PM (Every other Saturday and Sunday)

Shift 2: every Sunday 8AM -8PM

Pay Rate: $11.33/hourly Part Time

DDM is a Christian Ministry, and it is our purpose to provide residents with the highest quality of care and services in an atmosphere of dignity and respect.

Support residents in daily living skills according to the ISP and skill levels.

Maintain resident records according to DDM policy and be able to use electronic documentation for health records, ISP goals, community activities, daily activities and incident reports.

Minimum Requirements:

  • Minimum educational requirements specified in licensing regulations (currently high school diploma or GED). Must bring a copy of your diploma.
  • At least 21 (twenty-one) years of age.
  • Hold prior to employment a valid Georgia driver’s license (with no more than 2 driving violations in the past 3 years; no DUI or super speeder tickets).

DDM is a Christian Ministry and it is our purpose to provide residents with the highest quality of care and services in an atmosphere of dignity and respect.

  • Treat residents and co-workers according to the Golden Rule (Matthew 7:12): “Do unto others as you would have them do unto you.”
  • Support residents in daily living skills according to the ISP and skill levels.
  • Observe and follow all agency and state policies and procedures.
  • Attend staff educational meetings, webinars, and training.
  • Demonstrate person-centered service with the individuals served by integration in the community and completing notes using person-centered language.
  • Maintain DDM residence by completing safety drills and cleaning schedules. Report all facilities concerns through the secured email system to the Residential Services Coordinator and Residential Director.
  • Maintain DDM vehicles and observe all policies and traffic laws.
  • Supervise residents with self-administration of medications as prescribed and follow DDM policy concerning medications.
  • Schedule, transport, and become the residents advocate at medical appointments as needed and complete documentation in the electronic documentation system.
  • Maintain resident records as according to DDM policy and be able to use electronic documentation for health records, ISP goals, community activities, daily activities and incident reports.
  • Accounting for the management of petty cash and resident’s personal money.
  • Demonstrate the ability to create healthy menus, grocery shop if assigned, prepare meals, according to menus, and properly store and label food.
  • Audit and understand the documentation required for the individuals served.
  • At all times respect and treat individuals served as adults who have rights and responsibilities.
  • It may be necessary to perform tasks of a nature requiring supporting residents with equilibrium problems, lifting, stooping, bending, and climbing stairs.
  • Maintain contact with Residential Services Coordinator and Residential Director (if needed) and report any significant events immediately including concerns from families.
  • Demonstrate professionalism and contact immediate supervisor when:visited by any state agency

presented with concerns by family and day programsvisited by support coordination

  • Complete training schedule as noted in the Employee Accountability Checklist.
  • Maintain confidentiality regarding residents and other staff members as required by HIPAA.
  • Ensure that all individuals are supported and can explore and express their interest for community membership.
  • Any other task as assigned that provide supports for the individuals served.
  • Demonstrate flexibility and willingness to go beyond specific limitations of job description.

Pay: $11.33 per hour

  • Day shift
  • Rotating weekends
  • Weekends only
