German Centre for extened care is serching for a seasoned Social Worker Director to join our team. For over 100 years the German Centre, a non-profit organization that provides skilled nursing care with a certified Alzheimer's unit,an Assisted Living community and Adult day care all on one campus. Our community contiues to be amongst the top rated senior living communities in the state of Massachusetts.
Reports to the CEO/NHA
Qualifications:
- Licensed Social Worker
- Two plus years of experience working in a skilled nursing facility
- Ability to multi task duties
- Must have knowledge of Dept. of Public Health regulations
- Have a working knowledge of completing PASSR
- Understands Advanced Directives requirements.
Duties Include
- Coordinates interdisciplinary team care plan meeting
- Facilitates discharge planning for short term stay residents with family.
- Knowledge of community resources and arranges support services for residents safe discharges.
- Liaison for family members
- As a department head participation in quality improvement meeting is required and necessary to report department orperating satus.
- Oversees and directs assistants social worker.
