Detroit Wayne Integrated Health Network is Hiring a SUD Health Home Administrator Near Detroit, MI

Under the general supervision of the Health Home Director, the DWIHN SUD Health Home Administrator is responsible for the overall design and ongoing implementation of the SUD Health Home program. This position also provides oversight, management, and coordination for all activities within the SUD Health Home program.
Principal Duties And Responsibilities
  • Coordinates all enrollments into the health home with providers.
  • Coordinates with Lead Entity care management staff and Health Home Partners to identify a beneficiary’s optimal setting of care.
  • Coordinates and utilizes HIT with the Health Home team to maximize care coordination and care management.
  • Serves as a liaison between the health home sites and MDHHS staff/contractors.
  • Implements practice transformation based on health home principles.
  • Ensures person-centered and integrated recovery action planning.
  • Coordinates and integrates all clinical and non-clinical health care related needs and services.
  • Ensures Health Home services are delivered in a quality-driven, cost-effective, and culturally competent manner that addresses health disparities and improves health literacy.
  • Develops and maintains working relationships with providers and community partners.
  • Collects and reports on data that permits an evaluation of increased coordination of care and chronic disease management.
  • Monitors Health Home performance and leads quality improvement efforts.
  • Designs and develops prevention and wellness initiatives and referral tracking.
  • Serves as a member of the DWIHN Clinical Team.
  • Works collaboratively with Directors of the units within DWIHN to meet the overall goals and objectives of DWIHN.
  • Serves as DWIHN’s liaison to MDHHS SUD Health Home Division.
  • Provides consultation, training, general supervision, oversight and technical assistance across systems for Health Home initiatives.
  • Convenes collaborative workgroups to resolve issues and/or barriers.
  • Analyzes the service delivery system and makes recommendations to DWIHN regarding service needs.
  • Reviews funding needs in support of services in consultation with the Chief Financial Officer and makes recommendations for Board Actions.
  • Works with local and regional Universities and their staff to support Research and Evaluation activities aligned with Health Home Initiatives.
  • Creates presentations for state and national meetings related to Health Home initiatives.
  • Supervises program and support staff as assigned.
  • Develops and monitors productivity and efficiency standards.
  • Provides training and technical assistance.
  • Represents DWIHN at forums as assigned by the Health Home Director or their designee.
  • Performs related duties as assigned.

  • Judgement/Reasoning ability
Required EducationA Master’s Degree in Social Work, Psychology, Counseling, Health Administration, or a Human Service or Social Service field;
ORA Bachelor’s Degree in Nursing.
Required ExperienceFive (5) years of progressive post-degree experience in mental health service delivery to individuals with substance use disorders.
Work history must have included direct clinical work experience.
ANDAt least one (1) year of direct experience working in community-based programs or agencies that provide services for persons with substance use disorders.
Preferred License(s).Valid State of Michigan clinical licensure: RN, LMSW , LMHC, LPC, LLP or PhD.
Working ConditionsWork is usually performed in an office setting but requires the employee to drive to different sites throughout Wayne County and the State of Michigan. This position can work remotely with supervisory approval. Currently this position is primarily a remote position.
