Desert Oasis Healthcare is Hiring a Clinical Resource Specialist I Near Palm Springs, CA
- The position will maintain a revolving case load of 30 members that need to be evaluated for special programs within the organization.
- The CRSI utilizes clinical judgment, independent analysis, critical thinking skills and detailed knowledge of medical policies, clinical guidelines and plan benefits to achieve smoother transitions of care along the healthcare continuum.
- Assist with the development and implementation of care plan for members. This development may occur both during and outside of formal Interdisciplinary Care Team (ICT) meetings. It is the expectation that the CRSI will attend these planned meetings at least monthly.
- The CRSI will document and communicate effectively Problem, Intervention and Plan within the current Electronic Medical Record. This documentation must meet the needs of the medical group by supporting the requirements mandated by the health plans in order to meet routine audits.
- Act as a resource to medical group personnel and provider network in identifying available community resources and clinical services program options.
- Act as the liaison between the Member, the Primary Care Physician and others within the Members ICT. Helping to ensure that necessary information is shared to facilitate well-informed health care decision making and care plan goal attainment.
- Addressing with Members during interactions the importance of preventative care and the need for appropriate follow up with those Health Care Professionals rendering health care with focus on needed quality measures.
- Provide outreach to new members to medical group in order to assess healthcare needs as assigned.
- Provide informational services to those assigned members with focus on points of entry into health care services. (i.e. Primary Care Provider, Immediate Care, Member Services).
- Perform other duties as assigned.
- Current California RN License.
- A minimum of 5 years recent acute hospital experience, including experience with telemetry as well as a broad knowledge of the various nursing disciplines. Minimum 2 years previous experience in management or leadership role preferred. Prior experience in chart review and analysis required.
- Must possess an extensive understanding of varied diagnostic and invasive procedures and the ability to anticipate care plan changes based on their findings.
- Ability to communicate professionally and interface assertively with Physicians.
- Must be detail oriented and able to continually re-prioritize multiple tasks.
- California driver's license and proof of automobile insurance.
- BLS required within 60 days of hire.
