Position Purpose: This is a hybrid position, with 40% of the time dedicated to administrative responsibilities and 60% to direct patient care.
The position requires a 1.0 FTE
Essential Duties:
Direct Pt Care/Clinical duties
Administrative Duties
- Achieves productivity standards.
- Proficiently completes all clinical tasks and activities as outlined in privileging form.
- Maintains evidence-based practice.
- Supervises and coordinates all primary care patient activities.
- Directs and oversees primary care Telehealth services.
- Follows timely and complete EHR documentation expectations.
- Assumes responsibility for patients that have a history of special and complex care needs.
- Seeks consultation and initiates specialty referrals as clinically indicated.
- Directs and oversees internal and external referral processes.
- Directs and oversees clinical crisis management.
- Practices safety, environmental, and infection control
- Serves as resource for medical team.
-Colloborates with Director of Dentistry for infection control, and clinical coordiantion.
- Leads implementation of Medical policy, grant deliverables, and contractual obligations.
All primary care services, including onsite labs, are provided at the direction of the Primary Care Director and in the context of supporting integrated care and PCMH.
Oversees all departmental personnel management. These responsibilities include but are not limited to:
- Recruitment, hiring and training of new staff
-Seeks guidance from executive leadership as needed.
- conducts annual staff performance review and this review includes overseeing peer review and reappointment of privileges
Assumes responsibility for day-to-day operations. These responsibilities include but are not limited to:
- directing the assignments of and supervising personnel, students, and volunteers
- maintaining compliance with state, federal, and OSHA regulations
- oversees scheduling of staff and patients
Oversees referrals to specialty services in order to meet patient needs that exceed the practitioner’s scope of practice and limitations of the facility.
Collaborate with grant manager for ongoing and new grants if applicable.
- Manage and meet grant deliverables for program area if applicable.
Attend committee meetings, staff meetings, and other meetings as assigned.
- Member of quality improvement committee
Writes and maintains policies and procedures in the program area.
Maintains a work environment marked by respect for others and in accordance with DIHFS mission.
Has a clear understanding of providing services to underserved populations.
Cultural Competency a must.
Oversees primary care data collection and reviews it periodically to identify trends, outcomes, and opportunities.
Other Duties

Collaborates with behavioral health, dental health, pharmacy, and executive team
Other duties as assigned by the executive team
Job Qualifications
Be willing and able to remain credentialed by health plans and ensure credentialing materials are current.
Be willing to ensure strict compliance with DIHFS, IHS, professional, ethical standards, and legal requirements.
Must be willing and able to work in a patient care environment with exposure to biological hazards and infectious diseases.
Be willing and able to supervise the treatment of care across the lifespan.
Be willing and able to take clinical/administrative calls after hours.
Be willing and able to participate in telehealth calls with patients.
Be willing and able to promote Patient-Centered Medical Home principles.
Be willing and able to lead teams to achieve strategic goals.
Be willing to abide by DIHFS employee immunization requirements.
Education, Licensure or Certificate
Graduate from an accredited program in school of Medicine or Nursing.
Valid NPI number.
National board certification and expertise in practice area
Prescriptive authority with DEA License in good standing
By Date of Hire:
- Licensed MD, DO, NP in good standing with Colorado Department of Regulatory Agencies (DORA)
- Colorado Medicaid Provider number
- Completed CAQH
- Current BLS/CPR Certification
Experience
Minimum of 5 years of clinical experience
Previous healthcare program management experience and/or supervisory experience.
Preferred Qualifications
- Experience with Native American clients.
- Experience with population-based care
- Experience in Integrated Health care setting
