Job Summary:

Under general professional supervision, preforms behavioral health social work services including planning, organizing, treatment and inpatient behavioral health discharge/disposition. Delivers social services in accordance with the approved plan of treatment and plan of care. Assists the physician and other behavioral healthcare members in understanding significant social and emotional factors related to the patient’s behavioral health problems, including any substance use disorder diagnoses.

Knowledge, Skills & Abilities:

• Knowledge of emotional and social factors that affect patient’s health.
• Knowledge of discharge and in-service education programs.
• Excellent interpersonal skills.
• Skilled in identifying problems/opportunities to improve care. Ability to help the patient and family to understand, accept and follow medical recommendations.
• Ability to utilize resources, such as family and community agencies to assist the patient to resume life in the community.
• Ability to collect key information from the patient, family and the community as part of the social worker assessment.
• Ability to communicate to the appropriate staff information about the patient’s discharge plan.
• Ability to manage time sufficiently to achieve established goals.
• Skill in obtaining appropriate behavioral health care resources/services and educating others.
• Ability to articulate the professional use of self in the relationship to maximize client’s adjustment, enhance strengths and promote resolution of identified problems.
• Ability to develop effective therapeutic interventions that are socio-culturally sensitive.

Education: Master’s Degree in Social Work from a CSWE accredited college or university required.

Experience: One year of experience required.

Minimum: 58,900.00

Midpoint: 72,200.00

Maximum: 85,500.00
