Denton County MHMR Center is Hiring a Mental Health Case Manager Near Denton, Tx

Under direction of the ProgramManager of Adult Mental Health and within federal, state and local guidelines performs routine case management work. Work involves developing and maintaining long-term contact withclients, client families, and service providers for medical, social,educational, and related service needs.

This position is responsible for overseeing the care of individualsserved assigned to various Levels of Care (LOC). Works to ensure the continuityof care for individuals served. Works under moderate supervision, with limited latitude for the use ofinitiative and independent judgment.

Education,Training and Experience (including licensure and certification)

Graduation froman accredited four-year college or university with major course work in a humanservices field as required to qualify as a Qualified Mental Health Professional(QMHP) and six (6) months prior experience in a mental health field preferred.Master's Degree in a human services field and previous Crisis experiencepreferred. English/Spanish bilingual preferred.

REGISTRATION, CERTIFICATION,LICENSURE OR OTHER QUALIFICATIONS:

  • Individual needs to meet the requirements to qualify as a QMHP (Qualified Mental Health Professional).
  • All job specific training is required to be completed within 30 days of hire.
  • This position requires a valid Texas driver's license, auto liability insurance and a driving record acceptable to the Centers insurance requirements.

· The position requires adherence to all applicable laws,regulations, policies, procedures and/or guidelines of HHSC and DCMHMR. Other specific laws, regulations, policies,procedures and/or guidelines which must be followed include: TexasAdministrative Code and Case Management Guidelines.

Knowledge,Skills and Abilities

· Knowledgeof community resources, of case management delivery systems, and of programpolicies and procedures.

· Ability to assess clients' needs,coordinate individuals' services, communicate effectively, and to train others.

· Ability to work as a member of aninterdisciplinary team to ensure referral to appropriate Center program asneeded.

· Hasthe ability to work independently and make decisions but seeks guidance whenneeded.

· Ability to make decisions based oneducation and or clinical experience, which reflect consistency with Center'sGuiding Principles, Policies and Procedures, and Standards of Care.

· This position requires an understandingof community resources available for Denton County and the willingness toacquire new resources in the community.

· Understanding and practice of TraumaInformed Care principles.

· Ability to work with clients ofdifferent cultural backgrounds, sexual orientation, and belief systems.

· Ability to work with clients who maydisplay symptoms of mental illness including; poor hygiene, difficultycontrolling anger and low frustration tolerance, difficulty or inability todisplay appropriate social behavior, and who are suicidal or homicidal.

Essential Duties

1. Attendsand participates in all scheduled team meetings.

2. Providescourteous, professional services at all times.

3. ProvidesCase Management services. Caseloadsize will vary.

· Providesupported employment, supported housing, budgeting, skills training asappropriate, etc., to individuals served as defined in Texas Resilience Recovery(TRR) guidelines.

· Providesresources/referrals to meet individual needs and helps them navigate differentsystems.

· Willassist other teammates in their absence by providing services to individuals ontheir caseload as needed.

4. Completescommunity base visits per minimum monthly contact.

5. Providestransportation for individuals as needed.

6. Ensureseach individual served is provided services as required by the level of careassignment.

7. Administersclinical assessments and paperwork to determine appropriateness of level ofcare assignments and level of rehabilitative services required to meet individual'sneeds.

8. Isfamiliar with all applicable Texas Administrative Code (TAC), fidelity tools,and Texas Human Health Services (HHSC) contract information and Center policiesand procedures.

9. Ensuresindividuals charts are in compliance with HHSC contract requirements andstandards.

10. Providesemergency/crisis intervention when necessary according to HHSC and Centerpolicies and procedures.

11. Completesother duties as assigned.
