Delta Health Alliance is Hiring a Clinic Nurse Near Leland, MS

This person is responsible for assisting in the delivery of clinical services and health education associated with the patient-centered medical home. This individual will assist in the provision of nursing services to patients with chronic illness, provide patient-based educational programs, and assist in developing and sustaining patient-centered medical home services.

MINIMUM REQUIREMENTS: 

  • High school or GED equivalent.
  • Certification as a Licensed Practical Nurse (LPN), Certified Medical Assistant (CMA), or Certified Nursing Assistant (CAN) and Phlebotomy Certification preferred.
  • Minimum of three years’ experience in the delivery of healthcare programs.
  • Valid driver’s license and insurance

RESPONSIBILITIES: 

  • Prepares patient of all ages for provider office visits which includes: medication reconciliation, reviewing preventative services due, vital signs, immunization administration, and collecting pertinent health, social and lifestyle data.
  • Ensure that patients and exam room are prepared for the provider based on the information gathered in triage. Patient should be appropriately draped, and all supplies and equipment required by the provider should be in the room ready for use.
  • Under the direction of the provider, is responsible for required nursing procedures including: medication administration, dressings/wound care, specimen collection, in clinic lab tests, ear washes and other nursing procedures.
  • Ensures the timely flow of patients in the clinic. This includes triage of patients within 10 minutes of appointment time, timely delivery of nursing and lab related services and communication of results to provider and patients while in the clinic.
  • Patient education regarding both acute follow-up care and health education related to chronic care conditions.
  • Serve as the primary communicator between the provider and patients between visits. Timely return of phone calls and proactive communication with patients post visit.
  • Timely and accurate management of the EHR including:
    • Processes all referrals for services outside the clinic including authorization, scheduling and ensuring results are included in the patient medical record.
    • Conducts pre-visit planning by reviewing and updating patient charts prior to appointment with recent test results and correspondence.
  • Participates in the development of the plan of care for individual patients in consultation with the healthcare team.
  • Provide guidance and assistance in meeting HIPAA, State Department of Health and other healthcare regulations.
  • Maintain sample medication records, prepares and maintains clinical supplies and equipment.
  • Assists with scheduling tests, treatments, and procedures and processing referrals to other providers.
  • Provide basic lab services and blood draws
  • Maintain compliance with all local, state and federal clinic regulations
  • Assist in recruiting patients to the LMC
  • Follow all procedures related to supporting the Patient Centered Medical Home recognition.
  • Other duties as assigned.
