DaVita Integrated Kidney Care (DaVita IKC) is the integrated care division of DaVita Inc. working on DaVita’s vision to provide integrated care to all ESRD patients, who are some of the most medically complex and vulnerable patient populations in the US. Our more than 600 dedicated nurses, care coordinators, nurse practitioners (NPs) and business professionals integrate and manage care for more than 20,000 patients with late-stage chronic kidney disease (CKD) and end-stage renal disease (ESRD) across the US each month.

We have a great opportunity on a newly created role as a Regional Patient Enrollment Specialist. This exciting opportunity will travel weekly to DaVita clinics within the greater Cincinnati Market.

The Regional Patient Enrollment Specialist will assist with the scheduling and completion of annual telehealth evaluations, including introducing the patient and clinic team to evaluations, bringing all required technology, scheduling and supporting the overall evaluation. The Regional Patient Enrollment Specialist will act as the point of contact between the clinics and IKC. They support IKC/DKC collaboration by providing an overview of Integrated Kidney Care (IKC) programs to clinic patients, providing education materials, holding clinic appreciation events, educating telehealth leads on best practices and building clinic relationships to assist in navigating their local IKC teams. The Regional Patient Enrollment Specialist will work closely with the Engagement Specialists and Registered Nurse teams to ensure close collaboration of patient needs, supporting patient care goals and outcomes.

Responsibilities include of the Regional Patient Enrollment Specialist, but are not limited to:

  • Collect and share data as needed to assist the Nurse Practitioners and other IKC members’ patient records in the electronic health record
  • Provide technical and administrative support to clinics, patients and nurse practitioners including reviewing the use of iPad for telehealth and providing high-level troubleshooting for iPad, EHR or other tech issues clinics may be experiencing
  • Maintain an organized and accurate schedule for all NPs, care teammates and patients by coordinating with the clinics and booking/rescheduling patients for telehealth evaluations
  • Respond to clinic and patient needs in person or via telephone by routing or escalating to local teams or leadership as needed
  • Act as a liaison between patients, clinics and IKC as needed and help support IKC and field teammates
  • Achieve metrics and goals as it relates to telehealth evaluations, IKC collaboration or IKC contracts

Education & Experience Requirements of the Region Patient Enrollment Specialist:

  • Associate’s Degree or Higher
  • At least 2 years experience in a healthcare setting
  • Dialysis experience, preferred
  • Proficient in Microsoft Office suite
  • Strong written and verbal communication skills
  • Valid Driver’s License and Proof of Insurance
  • Travel weekly to clinics within the market
