Davis Behavioral Health, Inc. is seeking a full-time Case Manager/Social Service Worker (SSW) to provide case management services as a member of the adult mental health team housed in Layton, Utah. The successful candidate will work under the supervision of the team supervisor and will assume case management responsibilities for an assigned number of consumers and participate in the comprehensive assessment/treatment planning process while working with committed adults and clients with severe mental illness and addiction issues. The successful candidate will be involved in the provision of treatment, rehabilitation, and support services to help clients remain living independently in the community, including shopping, basis living skills, daily living skills, obtaining benefits, delivering medications and other duties as assigned. This position will also involve outreach in the community and interaction with all adult programs across DBH.
The successful candidate will work as a liaison between DBH and the local housing authority to assist with housing client housing needs.
Schedule
This position is classified as full-time, non-exempt and will work 40 hours per week.
The schedule assigned to this position is outlined below will consist of mostly day time hours, but require flexibility to meet the needs of the clients on the case manager's caseload.

Experience and Skills:
Candidates licensed as a Social Service Worker (SSW) or a Licensed Practical Nurse (LPN) in the State of Utah are preferred. Candidates who are not currently licensed but are actively pursuing SSW licensure or those that do not have a SSW license, but are eligible for Targeted Case Management Certification may also be considered.
Davis Behavioral Health utilizes an electronic medical record, which requires proficiency in computer-related skills.
Meeting monthly face-to-face service expectations and productivity standards is also required.
The selected candidate must successfully pass a pre-employment criminal background check and drug screening. DBH will not hire individuals currently using illegal drugs or abusing alcohol; therefore, making employment at DBH conditional upon successfully passing pre-employment drug screening by providing a valid sample. Candidates who are offered employment are required to report for a drug screen at an Intermountain WorkMed location within 24 hours of being requested to do so by the DBH Human Resources Department.
Davis Behavioral Health works with Federal grants and must comply with the Federal drug laws, including no use of medical marijuana.
Targeted Case Management Certification
Candidates who do not hold a professional license must complete certification as a Targeted Case Manager (TCM) within the first 30 days of employment.
Qualifications to become a certified Targeted Case Manager through the State of Utah Division of Substance Abuse and Mental Health include:
Preference will also be given to candidates who are bilingual English/Spanish.
The successful candidate must also have a clean driving record to operate agency vehicles and/or transport clients, when necessary. The ideal candidate will have experience working with clients that have sever mental illness, are hard working, can work both independently and successfully within a small outreach team.
