General Job Summary:

Responsible for helping with all aspects of patient care, including diagnosis, treatments, and consultations for patient population, including performing duties associated with the value-based care model.

Primary Job Responsibilities/Tasks:

1. Examine patients, diagnose illnesses, provide treatment and prescribe medications

2. Perform Medicare Annual Wellness Visits as required.

3. Order, interpret, and evaluate diagnostic tests

4. Gather health histories, perform physical examinations and diagnostic procedures

5. Develop and implement treatment plans

6. Dictate and document all patient interactions

7. Reconcile hospital discharge medications and contact patients as required

8. Contact patient discharged from the ER and triage as necessary

9. Complete FMLA fand DME forms as needed

10. Participate in value-based contract compliance activities

11. Notify managing physician about unusual or unexpected events or problems

12. Effectively communicate with diverse populations, including physicians, employees, patients, and families

13. Other duties as directed by managing physician

Job Types: Full-time, Part-time

Pay: $90,000.00 - $100,000.00 per year

Expected hours: 38 – 40 per week
