Founded in 1987, Damien Center is Indiana's oldest and largest AIDS service organization (ASO) and serves more than 8000 individuals living with or at risk for HIV through a comprehensive, innovative approach to care and prevention. Position Summary:
The PrEP Navigator will provide indirect service by assessing eligibility, offering insurance assistance, implementing a case management approach, and educating clients seeking PrEP care through Damien Center's PrEP services. The program's goal is to provide sexual health education to communities, at high risk of HIV infection, about the availability of PrEP services, as well as HIV/STI testing. The goal of this position is to engage new and established patients through their PrEP care continuum.
Duties and Responsibilities:

• Maintain an active caseload of PrEP patients
• Provide support and guidance to all referrals seeking linkage to PrEP, PEP, STI testing, and other medical providers
• Performs assessment, plan creation, and assisting patients to ensure access to medication and adherence to treatment plan, as well as risk reduction plan
• Assist clients as they move through the service delivery system, including following-up with patient that have fallen out of service
• Collect necessary documentation to navigate patients through the necessary steps to acquire insurance, as needed.
• Assist with the elimination of barriers, including but not limited to financial and transportation barriers
• Provide counseling or appropriate referrals to both internal and external services
• Advocate and act as a liaison between the patient and all relevant professionals through the PrEP program, that includes Clinical, Prevention and Pharmacy staff
• Educate clients and others about the HIV disease process and prevention
• Complete necessary paperwork and data entry in a timely manner including entering case notes within 48 hours
• Support the Tele-PrEP Navigator with new and established patients, as needed
• Collect necessary documentation
• Maintain client records
• Assist in the promotion of the program at local health fairs and events, as well as implement other recruitment strategies for the program.
• Ability to communicate well with medical providers and support staff
• Ability to interact successfully with diverse populations
• Perform other duties as assigned by leadership
Education and/or Experience
Bachelor's degree preferred, but not required. Experience with Medicare/ Medicaid and commercial Insurance preferred. Experience working in a medical setting preferred. Bilingual in Spanish/ English a plus.
Work experience may substitute for education requirements on a case-by-case basis.
Knowledge, Skills, and Abilities:
Possess superior organizational and administrative skills. Excellent interpersonal and communication abilities. Excellent computer skills in a Microsoft Windows environment. Ability to work independently and achieve high standards of productivity, achieving deadlines and with superior product results. Strong calendaring, organizational, and document management skills required. Proactive approach to solving problems and communicating to leadership. Knowledge of office management systems, procedures, and office equipment required. Excellent time management skills and ability to prioritize work.
