Description

DAMIAN FAMILY CARE CENTERS, INC. is a Federally Qualified Health Center with 14 community based health centers located in the metro NYC area, Suffolk County and Upstate, NY, providing primary care, behavioral health and specialized care at all sites.

Position Summary

Utilizes specialized knowledge and skills in performing health assessments, diagnosis and treatment of illnesses and chronic diseases. Provides advanced health care to people of all ages; prescribing treatment modalities, establishing plans of care and promoting the continuum of care with other health care providers.

May write prescriptions for medications without the co-signature of a physician, utilizing their own NYS license and DEA number.

Essential Functions:

Performs complete history and physical examinations in accordance with current standards of professional practice, regulatory requirements, and organizational policies.

Utilizes the nursing process, current standards of professional practice and standards of care in assessing patient needs and developing plans of care.

Demonstrates competency in diagnosing and prescribing appropriate treatment interventions for the primary care patient and assessing the patient’s response to prescribed treatment modalities.

Collaborates effectively with medical providers and other clinical disciplines in planning interventions that will best meet the patient needs in a timely manner. 

Appropriately prescribes medication regimens for the patient. Utilizes an outcome-oriented approach to assess patient’s response to prescribed regimens. 

Prescribes appropriate laboratory / diagnostic procedures and consultative referrals related to the patient’s health status and facilitates appropriate treatment interventions predicated on results of these procedures and consultative reports.

Identifies patient’s educational needs and utilizes appropriate resources to meet these needs.

Consults with the collaborating physician under the following circumstances:

· Patients who are acutely ill and may require hospitalization

· When deemed necessary by the CNP

· As requested by the Medical Director or collaborating physician.

Maintains open communication with all members of the health care team to promote a collaborative and collegial relationship within and between the ambulatory care centers and residential treatment facilities.

Collaborates and consults with residential drug facility staff and outside agencies for additional health services and discharge planning.

Demonstrates a commitment to maintaining an acceptable level of productivity by:

· Reporting to work on time and in accordance with assigned schedule;

· Facilitating the care of patients in a timely and efficient manner through appropriate triage of scheduled patients as well as walk-ins;

· Assisting other medical providers with heavier patient assignments and emergency situations;

· Demonstrates flexibility in scheduling to meet the needs of patient care and hours of operation.

Participates in the development of patient standards of care as requested by the Medical Director or DFCC Administration.

Documents patient interventions and outcomes within the EHR in accordance with current standards of practice and organizational policies.

Verbalizes and demonstrates knowledge and compliance to DFCC Adult Standards of Care and policies and procedures.

Performs within scope of practice, demonstrates awareness of legal issues, utilizes appropriate judgment and actions to reduce potential adverse consequences for patients, self and the organization.

Participates in meetings, committee activities, peer review process, and the DFCC QI program, as assigned.

Provides staff education on the health needs of patients, as assigned.

Assesses personal learning needs and demonstrates a commitment to professional growth by attending continuing education seminars and maintaining current knowledge of health care trends, standards, and new technologies. 

Performs efficiently in emergency situations; remains calm, uses good judgment and follows established protocols.

Provides a safe and secure environment for patients, family, staff and self through the appropriate application of established safety and infection control principles.

Maintains compliance to universal precautions, OSHA standards for blood-borne pathogens, and infection control policies.

Meets all organizational requirements for mandatory education at the time of employment and annually, thereafter.

Verbalizes knowledge of and demonstrates sensitivity to the patient’s bill of rights, maintains patient confidentiality at all times, and adheres to HIPAA Privacy and Security Rules.

Demonstrates initiative and competency in the utilization of Health Information Technology to perform essential job functions, which include but are not limited to (1) documentation in EHR, (2) transmission of EHI, (3) retrieval of EHI.

Demonstrates respect for the cultural and religious diversities and needs of patients, families and other care providers.

Creates and maintains an atmosphere of courtesy and respect towards patients, families and care providers.

Demonstrates a commitment to building a supportive, team oriented work environment and projects a positive image to the communities we serve.

Supports the mission and philosophy of Damian Family Care Centers, Inc.

Performs other duties as assigned and deemed necessary.

Requirements

Qualifications and Requirements:

· Graduate of accredited school of nursing. Masters’ degree in nursing. 

· Current and unencumbered New York State Registered Nurse license and New York State Nurse Practitioner.

· Current NYS registration to practice as an RN and NP

· Minimum one years’ experience as a practicing NP in a hospital required, ambulatory care setting preferred

· Current DEA and ACLS certification

· Minimum continuing education credits (CEUs) needed to maintain their advanced nurse practice license

· Understands and is able to articulate the concepts of an interdisciplinary team model with a focus on outcome measurements; ability to work collaboratively with diverse constituents including, but not limited to, staff, members of Damian’s leadership team, medical staff, patients and the community.

· Must be analytical with an attention to detail; stellar communication and interpersonal skills; sound problem solving and critical thinking skills

· Proficient with in Microsoft Office Outlook, Word; able to document care in an electronic medical record (EMR).
