Under supervision within state and locally developed programs and policies, identifies individual, family, and community problems and selectively uses individual or family casework, group methods, or community organizations to work toward the rehabilitation, prevention, or control of these problems.

1. Provide ongoing case management for families open for child protection services within Dakota County’s specialty treatment court
2. Work collaboratively with a multi-disciplinary team of professionals in supporting parents whose children have been removed from their care due to parental substance use.
3. Determine with the client, and in collaboration with agencies, the need, and level of service for identified clients and monitor contracted service providers.
4. Develop and implement all or part of a case plan (with the client when possible), defining initial and ongoing measurable goals and objectives.
5. Provide, arrange, and coordinate direct services and advocacy to clients and families impacted by parental substance use.
6. Facilitate, integrate, and mobilize community resources on behalf of the client.
7. Evaluate, document, and monitor progress toward goals and objectives.
8. Research, maintain, and apply knowledge of relevant policies, procedures, and statutes.
9. Complete agency, state, and federal required documentation as related to client activity according to mandated timelines.
10. Terminate activity with client or provider based on resolution of established goals or successful integration to community resources.
11. Provide regular updates to the Family Dependency Treatment court.
12. Monitor children’s safety and wellbeing in out of home placement.
