CrossWinds Counseling & Wellness is Hiring a Wraparound Facilitator Near Emporia, KS

The Wraparound Facilitator position provides quality waiver services through a strength-based approach to youth who have a serious emotional disturbance (SED) diagnosis, and to their families, in support of the child maintaining residence in a family environment, learning independent living skills, attending, and succeeding in educational endeavors, and preventing or reducing hospitalizations.
Essential FunctionsReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
  • Provides direct SED waiver services to children, usually 4 to 18 years of age, and to their families, in an effort to maintain stability and residence in a family environment.
  • Meets or exceeds the billable hour standard of client contact hours set for position.
  • Confirms eligibility and completes waiver initial assessments upon client’s referral to the SED Waiver program and completes closure procedures for clients who have terminated treatment services from the SED waiver program. Works in partnership with clients and other vested individuals to design and implement a comprehensive and individualized plan of care based on a strengths assessment.
  • Collaborates with the client and family to identify individuals who will create the Wraparound Team, consisting of members of the client’s family, other service providers, and community members who are involved in the child’s daily life. Develops knowledge of community resources and how to access these services for the benefit of clients.
  • Schedules and helps facilitates wraparound meetings to design and implement a comprehensive and individualized plan of care at the start of and throughout SED waiver services, to draft revisions to the plan of care when needed, and to discuss progress and needs of child.
  • Assists clients and their families to identify, secure, and maintain community resources.
  • Provides and coordinates necessary referrals and delivery of services among schools, community agencies, and within the organization to ensure that the client is receiving necessary services.
  • Collaborates with provider staff to ensure incoming referrals for SED Waiver services are appropriate and eligible.
  • Supports family members of seriously emotionally disturbed children receiving waiver services by assisting them in acquiring skills necessary to understand and address the needs of the child’s mental illness and treatment, coping mechanisms, and strategies for behavior management.
  • Coaches clients who will be transitioning to adulthood to develop effective independent living skills, by acquiring and improving self-help, socialization, and adaptive skills necessary to be successful in employment, housing, education, and community life. This is often in collaboration with the treatment team.
  • Maintains accurate and complete records of all client services, as defined by state licensing agencies, funding agencies, and Mental Health Center standards and requirements. Submits progress notes, treatment plans, client status reports, timesheets, and all work-related paperwork in a timely manner and in acceptable format as determined by agency requirements.
Work EnvironmentThis job operates in a community setting, as well as in a professional office setting. This role regularly uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

While performing the duties of this job, the employee is occasionally required to stand; walk; sit; handle objects; grasp and reach for items with hands and arms; manipulate hands and fingers; climb stairs; balance; stoop, kneel, or crouch; and talk or hear. The employee must occasionally lift or move up to 15 pounds. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Language skills are required, including the ability to read, write, comprehend, and speak the English language. This position also requires the ability to interpret written materials, write reports and business correspondence, and effectively present and respond to questions.
Position Type/Expected Hours of WorkThis is a full-time position. Days of work are Monday through Friday, and hours vary by client need.
TravelOccasional travel within the seven-county catchment area to provide SED waiver services is required.
Required Education And ExperienceBachelor’s Degree in psychology, social work, counseling, or related field. Must be 21 years of age or older.
Preferred Education And ExperienceA minimum of one year’s work experience in the human services field is preferred.
License or CertificationMust complete wraparound facilitation training online prior to delivery of service.
Additional Eligibility QualificationsAbility to work with persons with severe emotional disturbance, who without waiver services would require psychiatric hospitalization; possess demonstrated interpersonal skills and ability to react effectively in a variety of human service situations; ability to work independently and as a member of a therapeutic team; possess prior computer experience in Microsoft Office programs and basic computer fluency; a minimum of one-year work experience in the human services field is desirable, but not required.
