Job Title: Family Home Provider
Description:
A Family Home Provider (FHP) offers a community-based residential alternative service for adults with intellectual or developmental disabilities under the Support for Community Living (SCL) Waiver. The trained (non-related) FHP provides daily support and care to the individual living in their home.
Location: Boyd County and surrounding areas
Requirements:
- Age: 21 years or older.
- Education: High school diploma or GED.
- Enter into a contract as an FHP for Crossroads of Hope, Inc.
- Valid driver’s license with proof of current mandatory liability insurance for transportation.
- A home inspection must be conducted by management staff at Crossroads of Hope to ensure compliance with regulatory standards.
- Every adult living in the home must pass a drug and background check.
Responsibilities:
- Attend required meetings and training sessions.
- Follow Guidelines of the FHP-Resident Care Agreement
- Ensure a safe and clean living environment for the participant.
- Maintain comprehensive and updated records (MAR's, financial records, behavioral data, task analyses, etc.).
- Administer medication as prescribed.
- Arrange transportation for the participant when necessary.
- Assist the participant with personal care and special needs as required.
- Support the participant with his/her goals and interests.
- Aid with meal preparation and household chores if needed.
- Communicate effectively with the participant, the participant's family, the company's staff and any professional that provides supports to the participant.
- Foster a positive relationship of care and respect with the participant.
Compensation: $100/day per participant (1099 Contract Wages), plus $550/month in rent per participant.
Preferred Qualifications: Prior caregiving experience, particularly with individuals having intellectual and developmental disabilities is preferred, but not mandatory. Training will be provided.
