Description

Job Summary:

Housed in the Macon County Jail, the Restore/Renew Program Facilitator’s primary role is to support and provide direct oversight to detainees in recovery from substance use disorders. The Program Facilitator should serve as a role model, mentor, advocate and motivator to recovering individuals in order to help reduce relapse and promote long-term recovery. If in personal recovery, the Program Facilitator should maintain their own sobriety and maintain open and honest communication with their manager regarding relapse potential. The Program Facilitator must demonstrate the ability to foster professional relationships with inmates.

Position Status

This is a full-time, non-exempt position. The Program Facilitator reports to the Chief Behavioral Health Officer. The facilitator position is a Monday through Friday, first shift position, though supplemental positions may include evenings, overnights and/or weekend availability; as well as on-call rotation.

Qualifications


Duties/Responsibilities:

  • Responsible for intake of patients/residents into the RESTORE/RENEW Program
  • Facilitate group counseling focused on life skills and recovery
  • Provides education through facilitation of group counseling sessions to include topics of anger management, maintaining sobriety, life skills and re-entry.
  • Ensures compliance of detainees with rules and expectations
  • Immediately report concerns of any nature to director manager and jail command staff
  • Receives referrals, provides 1:1 support to detainees as warranted, and maintains and records all pertinent patient information in a secure and accurate manner
  • Functions as treatment team member for RESTORE/RENEW program.
  • Identify barriers to treatment success and assist in overcoming identified barriers for clients.
  • Identify detainees who may meet criteria for Case Management and/or Psychotherapy (i.e. high risk, problem-prone patients). Refer to case manager or therapist as needed.
  • Makes appropriate referrals and assists with connecting to community resources on behalf of detainees, if applicable.
  • Assist in transition of detainee to Crossing’s substance use treatment programming.
  • Maintains accurate documentation of interactions with detainees, including statistical information to track success of the program.
  • Participates in performance improvement activities, collaborations and committees as assigned.
  • Maintains a professional presence in the community as a representative of the agency and the behavioral health program.
  • Maintains compliance with all laws, regulations and policies as defined by Federal, State and any other regulatory body.
  • May be required to work across multiple sites, including the community care campus and the jail setting
  • Vigilant to safety concerns and takes appropriate steps to ensure both patient and staff safety
  • Under supervision, manages the care of detainees in collaboration with other health professionals and members of the healthcare team in order to make decisions about treatment

  • Willingness to pursue continuing educational opportunities and maintain compliance with professional licensure and/or certification renewal

Education and Experience:

  • Master or Bachelor Degree in Social Work, Psychology, Counseling, Sociology or a related field preferred
  • Other Substance Use related Certifications welcomed
  • Successful work history involving direct patient care is a plus

Physical Requirements:

  • Ability to work in conditions that include exposure to blood, bodily fluids and tissues, contagious diseases and/or other potentially hazardous materials
  • Moderate physical exertion including frequent walking, standing, bending, reaching, carrying moderately heavy loads (up to 25 lbs.) and climbing stairs
  • Ability to work within a jail setting
