Social Worker - Mission Point Personal CareCross Keys Village - The Brethren Home Community (CKV) is currently seeking a compassionate professional to join our Personal Care team! As a Social Worker working at CKV, we will provide you with opportunities to enjoy a friendly work environment guided by Christian values.
This opening is for a full-time position working Monday - Friday 8:00 a.m. - 4:30 p.m. with the ability to fluctuate hours as needed.

Personal Care, more commonly referred to as PC Mission Point, is designed for individuals who wish to remain independent when staying at home has become impractical or unsafe. Some of our residents might have limited mobility whereas others might be experiencing various degrees of memory loss. Regardless of their circumstance, Personal Care at CKV actively promotes residents aging in place. Our team members work together to create a more active and purposeful life for our residents, so they can lead healthier and more fulfilled lives.
Job Description:

As a Social Worker, your scope of responsibilities will include but is not limited to:

  • Assisting with the post-admission process in Mission Point Personal Care. Assisting residents and families with adjustment to Personal Care.
  • Interviewing and assessing social service needs of residents within 30 days of admission. Developing and implementing a plan to maximize psychosocial functioning and well-being for each resident. 
  • Assisting Accounting with collection of required documents for limited resource program. 
  • Assisting residents with application for VA benefits.
  • Assisting in supporting resident’s instrumental activities of daily living while pursuing guardianship.
  • Conducting cognitive screening.
  • Serving as a vital member of the Personal Care leadership team. Offering input on daily stand up meeting.
  • Providing grief support to the residents in Personal Care and their families.
  • Communicating and collaborating with hospice providers. Providing education to resident and family about hospice and makes referral to the clinical team.
  • Supporting CKV’s concept of “Aging-in-the-preferred-place,” identifies appropriate support systems, and educates the family about options of care.
  • Striving to ensure quality of life while assisting residents and families through transitions such as transfer, discharge, end-of-life, and death. Demonstrating strong communication skills and compassion while providing support to the family as well as to the team.
  • Assisting residents with emotional and psychological issues. Keeping the Personal Care Director of Clinical Services and resident’s responsible party updated, as appropriate. Seeking professional counseling or psychiatric services for residents as needed. Communicating with the provider to ensure residents are managed on the most effective form of treatment.
  • Completing comprehensive psychosocial assessments of Residents/Guests. Formulating, developing, and implementing interventions based on identified care needs.
  • Maintaining confidentiality around resident/guest and family information.
  • Providing admission support in the absence of the Admissions Counselor. Conducting tours of Mission Point Personal Care.
  • Maintaining a positive, professional relationship with hospital social workers and other referral sources.
Education and Experience:
 
  • Must have a bachelor’s degree in a human-services related field.
  • At least two years’ relative experience in a health care environment is preferred.
  • Must have working knowledge of relevant medical/legal issues that impact resident/guest care, e.g., Advance Directives, Resident Rights, etc.
  • Must possess a working knowledge of mental disorders and have an understanding of both psychosocial and pharmacological approaches to the treatment of emotional and behavioral disorders encountered most often in the long-term care setting.
  • Knowledge and understanding of regulatory standards, clinical terminology and meaning is helpful.
