Infection Prevention & Quality Assurance Director - Registered Nurse
Cross Keys Village - The Brethren Home Community (CKV) is currently seeking a skilled RN to coordinate all aspects of the Quality Assurance Performance Improvement (QAPI)/infection prevention and control program and provides direction for maximizing quality of care across the continuum. Job Description:
As the Infection Prevention & Quality Assurance Director your scope of responsibilities will include but is not limited to:
  • Coordinating and maintaining an effective Process Improvement Program (PIP) to address areas of opportunity based on QAPI data as well as regulatory compliance. Conducting and/or assisting leaders in the development of audits as measures of performance. Ensuring audits are performed in a timely manner. Working collaboratively with administrators and department leaders to identify areas for improvement, establish benchmarks, and set goals. Guides the QAPI team to implement proactive steps to achieve desired outcomes.
  • Overseeing the development of ongoing monitoring processes of clinical issues that have been identified in collaboration with the Director of Nursing within the Health Care Center Nursing Department. Responsible for the oversight of Process Improvement Plans related to clinical care in the Health Care Center. Collaborating with Nursing Administration to identify and develop systems and programs to assure quality and excellence. Assisting Nursing Administration in analyzing data to determine areas requiring systems development, changes, and/or improvement.
  • Overseeing QAPI-related programs such as the antibiotic stewardship program, antipsychotic utilization program, falls management program and other programs that are identified as high priorities in CMS Quality Measures and CASPER Reports.
  • Reviewing and reporting infection surveillance data and infection prevention activities to the QAPI Committee.
  • Providing best practices within the Health Care Center and across service lines as it relates to falls reduction, elopement, pressure ulcers other clinical care areas of significant risk.
  • Developing and implementing an ongoing infection prevention and control program to prevent, recognize, and control the onset and spread of infections in order to provide a safe and sanitary environment.
  • Establishing campus-wide systems for the prevention, identification, tracking, reporting, investigation, and control of infections and communicable diseases of residents, team members, providers, visitors, and volunteers.
  • Developing and implementing written policies and procedures in accordance with current standards of practice and recognized guidelines for infection prevention and control that are tailored to the facility. Reviewing and updating policies and procedures at least annually or according to state or federal requirements.
  • Staying up-to-date on CDC/HICPAC/COCA, CMS, DOH and other relevant guidelines and standards. Implementing infection prevention protocols in keeping with the best practices and evidence-based standards established by these governing bodies.
  • Overseeing infection control protocols for emergency preparedness purposes. Developing a written plan for outbreak response which includes definition, procedures for surveillance and containment, and a list of syndromes or pathogens for which monitoring is to be performed. Reporting to appropriate public health authorities and governing bodies regarding infectious outbreaks.
  • Responsible for overseeing and implementing all aspects of the vaccination programs in the Health Care Center and provides direction to other CKV service lines regarding vaccination standards and recommendations.
  • Overseeing the CKV-TBHC Infection and Prevention Control Committee. Developing action plans to address opportunities for improvement.
  • Implementing an annual infection control risk assessment process and participates in the Health Care Center's annual facility assessment review process.
  • Reviewing and/or revising CKV-TBHC's infection prevention and control program, its standards, policies, and procedures annually and as needed for changes to the facility assessment to ensure they are effective and in accordance with current standards of practice for preventing and controlling infections.
  • Providing education related to infection prevention and control principles, policies and procedures to team members, residents, volunteers, providers, and families (as appropriate.) Ensuring that supplies related to infection prevention are readily available in the appropriate locations (e.g., proper PPE), along with instructions. Conducting routine rounds to identify IP opportunities for improvement and follows up in a timely manner with related education, implementation, and ongoing evaluation.
  • Providing education in a variety of ways to effectively communicate with residents, team members, volunteers and visitors about infectious outbreaks and transmissible illnesses (e.g., Influenza, COVID, Tuberculosis, Hepatitis, respiratory pathogens) as well as risks associated with potential biological hazards (e.g., blood-borne pathogens). Conducting audits to promote best practices for infection control and prevention and provides feedback to improve processes as well as individual compliance with established protocols (e.g., hand hygiene, standard precautions, use of PPE).
  • Overseeing injection safety and point-of-care testing, which includes protocols for performing finger sticks and point of care testing (e.g., assisted blood glucose monitoring, or AMGB). Routinely conducting audits to determine adherence to injection safety procedures. In collaboration with Nursing Administration, ensures training and competency evaluation on injection safety.
  • Ensuring proper reporting of infectious illnesses to appropriate agencies and public health authorities as well as residents and responsible parties, as indicated.
  • Maintaining documentation of infection prevention and control program activities.
  • Providing ongoing risk and infection prevention consultation to the CKV-TBHC Benefits and Risk Manager as well as to service line administrators in Personal Care, Brookside, and Residential Living, as needed.
Education and Experience:
  • Must be licensed as a registered nurse in the State of Pennsylvania with a bachelor's degree in nursing preferred.
  • A minimum of 5 years professional clinical and progressive management experience.
  • Must demonstrate CPR competencies.
  • Must complete relevant training and certification in Infection Control, such as programs and exams developed by the Certification Board for Infection Control & Epidemiology
  • Must participates in infection control courses organized by the state or recognized professional societies such as the Association for Professionals in Infection Control and Epidemiology (APIC), and Society for Healthcare Epidemiology of America (SHEA).
