Crisis Services is Hiring a Domestic Violence Intensive Case Manager Near Buffalo, NY

POSITION SUMMARY: Carry a caseload of clients who have experienced domestic violence or elder abuse and also have co-occurring mental health or other high intensity needs. Provide crisis intervention, supportive counseling, case management, advocacy and follow-up services to victims of domestic, intimate partner, family violence, and elder abuse; provide comprehensive services in partnership with police, district attorney, and/or community-based agencies serving victims of domestic violence and elder abuse. The Domestic Violence Intensive Case Manager serves as a member of the Department's Case Management Team. This position will provide services on site at Crisis Services, at local police department(s) and/or in the community. The position requires mandatory on-call shifts in addition to regularly scheduled 40 hours/week. The regular hours are Monday-Friday. Final schedule to be determined, some evening and/or weekend hours may be required to meet client needs.

Essential Functions of the Position Include, But Are Not Limited to:

  • Maintain a commitment to the mission and work of Crisis Services with a deep appreciation for and sensitivity to the guiding principles of the organization.
  • Primary responsibility includes a smaller, high needs caseload of clients and assist them in navigating the criminal justice system and/or linkage to appropriate providers for co-occurring needs, for example, mental health or substance abuse, and/or needs related to elder abuse.
  • Provide Advocate Department Case Management services, including but not limited to: crisis intervention response, follow up and outreach to clients, supportive counseling, case management services, personal advocacy, linkage to community resources, assistance in obtaining NYS Office of Victim Services compensation, and assistance with criminal justice processes, and assistance with criminal justice and/or court advocacy for victims of domestic violence or elder abuse.
  • Perform follow-up site visits, in person, phone and possible virtual outreach to clients.
  • Provide follow up and case management services, including developing a care plan, linkage to additional community organizations, safety planning, enhancing skill building and coping strategies, as well as a reduction in victimization, increased level of functioning, improved capacity for independence, and reduction in level of isolation.
  • Communicate and coordinate with external community care providers to ensure successful client outcomes.
  • Responds to local county emergency rooms during regular hours when scheduled to provide crisis intervention and supportive services to survivors of domestic violence, sexual assault, family violence and/or elder abuse and/or provide support to Volunteers who are on call for the Hospital Response Program.

Hospital On Call: scheduled on call is required outside of the regularly scheduled shift covering the Department's 24/7 Hospital Response Advocacy services. If activated, may be required to respond to local county emergency rooms to provide crisis intervention and supportive services to survivors of domestic violence, sexual assault, family violence and/or elder abuse and/or provide support to Volunteers who are on call for the Hospital Response Program.

Position Qualifications:

  • EDUCATION: Master's degree from an accredited college and/or university in human service field, public health, social work or education
  • EXPERIENCE: Master's degree with minimum one (1) year experience, preferably in working family violence/sexual assault and/or mental health. Case management and/or direct client service experience required. Excellent skills in assessing levels of emotional distress and reacting appropriately. Demonstrated ability to work with people of various ethnic, cultural and socio-economic backgrounds, sexual orientation and gender identities. Awareness of best practices of risk reduction and survivor support strategies.
  • Must have a valid Driver's License and a clean driving record, access to reliable a vehicle and proof of auto insurance.
