Crisis Preparation and Recovery is Hiring a Transitional Care Clinician Licensed in Arizona Near Arizona, AZ

 
The Transitional Care Clinician is an integral part of the Transitional Care Program (TCP) and works to provide clinical support and counseling for clients in the program, while actively preparing them for transition to an outpatient provider and/or social and natural supports for ongoing services and stability. The position reports directly to the Divisional Operations Director. The position requires remote and mobile work throughout the Greater Phoenix Area and surrounding cities. Work 40 hours per week (8-hour and 10-hour shifts available).

Knowledge and Skills

  • Excellent oral and written skills to communicate in a professional manner

  • Strong computer literacy skills and ability to work within an electronic medical record system (EMR)

  • Knowledge and experience of evidence-based therapy interventions

  • Knowledge of community resources

  • Knowledge of the different levels of care within behavioral health

  • Knowledge of substance use disorders and appropriate levels of intervention, to include assessing for detox and MAT services.

  • Strong clinical skills to diagnosis and treat behavioral health issues

  • Ability to assess and monitor for danger to self/others risk

  • Knowledge of state-wide Behavioral Health Crisis Services

  • Excellent crisis management and de-escalation skills

Duties

  • Manage a caseload of clients to assist with transitioning to the appropriate level of care

  • Complete initial assessments, to include client’s current level of functioning, behavioral health symptoms and history

  • Determine DSM 5 diagnosis(es) and treatment needs

  • Provide ongoing counseling and care coordination

  • Create safety plans 

  • Complete treatment plans

  • Attempt engagement with clients at least twice weekly

  • Daily assessment of caseload to ensure required outreach is met, as well as discharges for inactive clients or clients nearing time-limit of program.

  • Review assigned measures and curriculum progress

  • Support clients with social determinant of health needs

  • Document all interactions and coordination of care efforts in the electronic medical record

  • Assist in the development of psycho-educational material as needed

  • Assist in training new members of the team

  • Develop and maintain working relationships with outside agencies/resources

  • For associate level clinicians, participate in weekly clinical supervision toward independent licensure

  • Other duties as assigned

Other Items

  • Provide evidence of professional liability insurance coverage in the amount acceptable, minimum $1/$3 million

  • All expirables to be renewed and submitted 30 days prior to the expiration date. It is the duty of the licensee to keep all expirables active and current and report any mandated change to the Board immediately.

  • All CPR clinical staff is required to apply for, and/or present and maintain, within 7 days of hire, a Class One Fingerprint Clearance card. If application is necessary, a copy must be submitted to include in personnel file until card is received.

Qualifications and Education

  • Master’s degree in counseling, social work, Marital and Family Therapy or related field required

  • Current, unrestricted license (LMSW/LCSW/LAC/LPC/PsyD) with the Arizona State Board of Behavioral Health Examiners (AZBBHE) required or successful attainment of license within 60 days of employment.

  • Crisis experience is preferred

  • One year of counseling experience preferred
