• Attend Morning clinical meeting and obtain information on residents or families with clinical concerns requiring follow-up.

• Follow up with new residents within three days of arrival.

• Make at least three round per day on every floor to discuss Residents needs and concerns.

• Utilize Saiva Report to round on residents with high risk for return to hospital.

• Respond to calls regarding clinical concerns or questions and communicating to nursing staff.

• Maintain a daily log with questions, concerns and issues raised; and any follow up steps taken.

• Submits Daily logs to the Facility Administrator.

• RN license required

• In order to be considered for employment, candidates must possess an active, unencumbered license to practice as an LPN or RN in this state.
• Ideal candidates will possess clinical experience in a sub-acute and/or long-term care setting.
