Crest Home Health is Hiring a Scheduling/ Medical Records Coordinator Near Boerne, TX

PRIMARY PURPOSE

In accordance with established policies and procedures, the Scheduling/Medical Records Coordinator is responsible for the coordination of patient visits by nurses, maintenance and upkeep of scheduling records and log books, and the accurate and timely communication of scheduling changes between office and field staff. To provide medical records administration for the Agency, by delivering orders to physicians for signature, tracking outstanding orders, maintaining patients’ physical charts, and auditing medical records for completeness/accuracy.

QUALIFICATIONS

Educational requirements:

1. High School Diploma

2. Preferred- Graduate of an approved program of vocational/practical nursing.

Experience/Skills Required:

1. One to two years of home health care experience.

2. Preferred - One to two years experience of scheduling experience.

1. Outstanding organizational skills.

2. Excellent phone skills; professional verbal communication.

3. Some medical terminology/medical records knowledge is desired.

4. Ability to type 45 wpm.

5. Effective ability to use Adobe, Microsoft Word, Microsoft Excel, etc.

6. Knowledge of common office equipment, such as fax, copier, shredder, printer, etc.

Physical Requirements:

3. Able to participate in light physical activity, including data entry/filing, light lifting, and extended periods of sitting or standing.

WORK ENVIRONMENT

Onsite office work.

RESPONSIBILITES:

  • Fax orders to physicians for signature.
  • Coordinate with marketing staff to obtain signed orders from physicians.
  • Keep track of all outstanding orders.
  • Receive and upload signed orders.
  • Audit paper and electronic medical records.
  • Flag deficiencies found in paper and electronic medical records.
  • Notify appropriate office staff/physician’s office staff of deficiencies.
  • Retrieve and assemble all discharged paper medical records.
  • Filing/maintain patient charts.
  • Answer phone with professional phone etiquette.
  • Communicates with clinicians/supervisors to resolve questions/issues.
  • Sort and scan all incoming faxes as appropriate.
  • Become knowledgeable of all company policies, regulatory rules, regulations and standards that pertain to protected patient information.
  • Maintains confidentiality of patient and proprietary information.
  • Coordinates and schedules patient visits.
  • Schedules appropriate skilled staff to care for clients after consultation with Clinical Coordinators based on activity and skill match.
  • Documents conversations with field staff and outside agencies and forwards memos to Clinical Coordinators/Director.
  • Completes missed visit reports based on reports from field staff.
  • Maintains an accurate hospital log
  • Attends staff meetings biweekly and as scheduled.
  • Communicates effectively with patients, their families, Agency staff, and external home healthcare providers and agencies.
  • Forwards all completed frequency grids to Director for review.
  • Maintains employee phone/beeper/cell phone/fax/email logs.
  • Performs other tasks as assigned.

Job Type: Full-time

Pay: $35,000.00 - $45,000.00 per year
