Long-Term Care Admissions Nurse Opportunity! 

Hilltop Skilled Nursing and Rehab Center is looking to add an Admissions Nurse to our growing team! 

The Admissions Nurse is an Interdisciplinary Team member whose primary responsibility is to present Hilltop Skilled Nursing and Rehab's services and philosophy to patients and families in a manner that results in the admission of appropriate patients. 

Your Job: 

  • Reviews all admission documentation in order to ensure the quality of the services to be provided
  • Participation in the information gathering, analysis, education and form completion aspects of the admissions process
  • Build effective referral source relationships to support the high quality and responsiveness expectations associated with Hilltop SNR
  • Assists with insurance authoriazations and contracts
  • Assures room readiness for new admissions 
  • Treat and care for residents and their property with dignity and respect
  • Responsible for the planning, coordination, and assurance of quality care services to residents
  • Utilize interpersonal skills to help appropriately educate and address any questions the patient and/or patient’s family has 
  • Provide nursing care within your scope of practice
  • Work cooperatively with healthcare team members to maintain established standards of care
  • Maintain the self-respect, personal dignity, and physical safety of each resident
  • Maintain accurate documentation of all care rendered
  • Encourage restorative care to obtain maximum resident independence
  • Any other job duties, as assigned by Administrator

Who You Are:

  • Current Licensed Practical Nurse or Registered Nurse in the state of Illinois
  • Graduate of an accredited school of nursing
