Creekside Post-Acute has been serving San Jose, California for over 50 years. It is a 130-bed that specializes in short-term rehabilitation and skilled nursing. We are looking for a committed, caring, and reliable Physical Therapy Assistant, PTA to join our in-house rehab team for our Skilled Nursing Facility!
All new hires must be COVID-19 Vaccinated or have a medical or religious accommodation before they start.
RESPONSIBILITIES:
Assist licensed physical therapists in evaluating and assessing patients' conditions and developing individualized treatment plans
Implement treatment plans under the direction of physical therapists, including conducting exercises and therapeutic activities
Monitor patients' progress and make necessary adjustments to treatment plans
Document patients' progress and maintain accurate and updated medical records
Communicate effectively with patients, their families, and other healthcare professionals to ensure coordinated and holistic care
Adhere to ethical and professional standards in all aspects of patient care
Stay up-to-date with the latest advancements and best practices in physical therapy
REQUIREMENTS:
BLS and Physical Therapist Assistant Certificate
Interest in working with the geriatric population
Is in good standing with all regulatory agencies and licensing boards is required
Associate’s degree or certification from a physical therapy program that included course study in anatomy and physiology, and clinical experience.
Strong understanding of human anatomy and physiology
Excellent communication and interpersonal skills
Ability to work effectively as part of a multidisciplinary team
Attention to detail and strong organizational skills
EXPERIENCE:
New passer/graduates are welcome
With Skilled Nursing or Long-Term Care experience is preferred
