Creative Pathways, Inc. is Hiring a Clinical Coordinator Near Waldorf, MD

Creative Pathways, Inc are searching for a Clinical Coordinator to work with our Foster Care youth in DC and Maryland. Must seek Dual licensure. Individuals will work from home majority of the time. However, they will have to go and visit the youth in their foster home on a weekly basis. 

Position Title: Clinical CoordinatorSupervisorClinical Manager

Position FunctionTo assist in the monitoring of foster care services delivered by Creative Pathways, Inc. through case management duties that include but are not limited to: Initial Treatment Plans, 90 Day Reviews, Contact Notes, weekly home visits, assist with foster parent in-service training.

Educational RequirementsA Bachelor’s or Master’s degree in Social Work. Maryland or DC licensure. CANS Certification. First aid, and CPR certification

Functional Requirements:Must have effective verbal and communication skills and the ability to work collaboratively with youth, biological and treatment families, referring agencies, community professionals, and Creative Pathways, Inc. staff.

Must demonstrate the following:

  • knowledge and ability to assess youth and family members for services.
  • the ability to objectively evaluate treatment foster parents suitability as caregivers.
  • an ability to continue learning through supervision, continuing education and experience and work cooperatively using a team approach.
  • knowledge of parenting techniques and an ability to work collaboratively with biological and treatment families to find solutions to problems, as well as crisis intervention skills.

Working Conditions:Position includes exposure to biological parents, guardians, as well as to children and youth with behavioral problems, including possible physical aggression. A personal vehicle is required for travel. This position requires a rotating on-call system.

Exposure to Confidential Information:Clinical Coordinator’s will have access to confidential records including youth files, foster care records, and treatment parent information. Must maintain confidentiality and follow policies related to personnel and client records.

Key Expectations/Responsibilities:

Planning:

  • Participate in program development.
  • Participate in CARF Committee’s
  • Participate in weekly supervision meetings.
  • Participate in treatment team meetings with collaborating partners and agencies.

Implementation:

  • Participate in treatment team meeting, court reviews, educational meetings as well as transfer and discharge meetings.
  • Documentation of meetings, contacts, and case progress by completing paperwork, contact/progress notes, and writing plans of service.
  • Facilitate meetings with treatment parents to review child/youth progress and overall functioning of the treatmentparents, as well as assist treatment parents in designing behavior interventions and responding to youth behavior.
  • Conduct inspections at foster homes to review licensing standards and document non-compliances.
  • Write clear and concise case records, letters and reports.

Training and Supervision:

  • Attend training offered at CPI to expand knowledge of services and the application of clinical skills.
  • Maintain first aid, and CPR certifications.
  • Communicate regularly with supervisor and seek supervision when appropriate.
  • Attend continuing education necessary to expand knowledge and maintain certifications or licenses.
  • Establish and adhere to a regular work schedule.
  • Notify supervisor properly when absent or otherwise unavailable.
  • Complete assignments and submit documentation in a timely manner.
  • Establish effective working relationships with clients and co-workers.
  • Maintain appropriate boundaries between professional and personal issues.
  • Adhere to all written policies and procedures of the agency and its accrediting and standard setting bodies, such as CARF and state licensing.
  • Respect the dignity and worth of clients and foster
  • Maintain client confidentiality per agency standards and all applicable codes of ethics.
  • Use outside professional collaboration and intra-agency teamwork appropriate to the agency.
  • Be prepared to address appropriate issues during supervisory conferences and meetings.
  • Accept direction from supervisor and implement supervisor’s suggestions and instructions.
  • Understand your job description in relation to other agency positions.
  • Consider the environmental, cultural, safety, diversity issues and needs for the treatment parent and client.
  • Complete other duties as assigned by supervisor.
