Job Description

The Cow Creek Band of The Umpqua Tribe of Indians is currently seeking a highly motivated and passionate individual to join our team as a Diabetes Lifestyle Coach. This position will play a key role in supporting individuals in managing their diabetes and adopting healthier lifestyle choices. Please note that this is Not a Remote Job.

Duties and Responsibilities

• Educate individuals on the importance of nutrition, exercise, and stress management in managing diabetes
• Develop personalized lifestyle plans for individuals with diabetes
• Lead group workshops and individual coaching sessions
• Monitor and track progress of participants
• Collaborate with healthcare professionals to provide integrated care for individuals with diabetes

Requirements

• Bachelor's degree in nutrition, health education, or related field
• Certification as a Diabetes Educator or willingness to obtain certification
• Excellent communication and interpersonal skills
• Experience working with individuals with diabetes
• Ability to work collaboratively in a team environment
