Summary:

This individual is responsible for skilled services under the supervision of a Registered Nurse (RN). The Hospice Weekender LPN will receive and coordinate information from incoming calls, address hospice clinical needs and visits three days weekly from 7a-7p. This includes every Friday, Saturday and Sunday. This position must consistently exercise discretion and judgment and practice in accordance with the plan of care and with the agency policies: prepares clinical progress notes, assists the RN in performing specialized procedures, prepares equipment and materials for treatments and assists the patient in learning appropriate self-care. The RN delegates skilled nursing functions after evaluating the complexity of the nursing activity and the level of problem solving required. Demonstrates excellent customer service performance in that his/her attitude and actions are consistent with the standards contained in the Vision, Mission and Values of Covenant HealthCare and the commitment to Extraordinary Care for Every Generation.

Responsibilities:

• Provides skilled nursing care to patients in accordance with the plan of care under the supervision of the RN. • Performs delegated nursing activities which are appropriate to the LPN’s preparation and experience as evidenced by skills checklist and in accordance with agency policies and program guidelines. • Implements the plan of care, following the RN’s initial assessment and development of the plan of care, utilizing practice standards. • Conferences with the RN on a regular basis, collects specific assessment data, participates in planning and evaluation of the nursing care plan as appropriate. • Provides for emotional and physical comfort and safety of patients. • Recognizes and understands the effects of social and economic problems of patients. • Maintains records in accordance with agency requirements. • Oasis information will be collected as mandated federally following agency guidelines. • Works with Home Health Aides and other disciplines as appropriate. • Collaborates and communicates with RN Weekend Facilitator on regular basis. • Participates regularly in agency staff meetings, team, meetings, and in-services. • Participates on agency committees or special task forces as assigned. • Participates in development, revision, and implementation of policies and procedures. • Assumes responsibility for personal and professional growth. • Maintains confidentiality of patient and agency information.

EDUCATION/EXPERIENCE REQUIREMENTS • Current Licensure with the Michigan State Board of Nursing. Graduation from a school of practical nursing. NAPNES recognized. • BLS required. • Two years nursing experience following graduation or at the discretion of the Program Services Supervisor. KNOWLEDGE/SKILLS/ABILITIES • Knowledge in current nursing practice, ability to relate positively to patients, families and co-workers. • Interpersonal skills in order to communicate effectively. • Successful ability to independently access any type of a private residence. • Valid Michigan driver’s license, reliable transportation and proof of auto insurance required. WORKING CONDITIONS/PHYSICAL DEMANDS • Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards. • Frequent standing, walking, sitting, climbing, kneeling, twisting, reaching, handling, feeling, and talking. • Frequent near vision, midrange vision, far vision, depth perception, visual accommodation, clor vision, and field of vision. • Frequent lifting 0 to 25 lbs. • Occasional lifting, pushing, pulling, climbing, balancing, stooping, crouching, and squatting. • Occasional lifting 26 to 100 lbs. or more.
