JOB SUMMARY
·Responsible to Rehabilitative Services Director In/Outpatient.
·Provides physical therapy evaluation and treatment to patients upon appropriate medical referral and/or as medically necessary.
·Performs related duties of documentation, communication with physicians and other ancillary services and patient charges.
·Participates in department meetings, in services, audits, Performance Improvement, and discharge planning when appropriate.
·Processes confidential information and adheres to Patient Bill of Rights.
·Provides holiday and weekend coverage as needed.
·Will provide coverage in other physical therapy departments as needed and determined by Director of Rehabilitation Services and immediate supervisor
JOB REQUIREMENTS
Education:
·Graduate of an accredited program in Physical Therapy with at least a Bachelor of Science Degree.
·Licensed in the State of New Hampshire prior to beginning employment.
Other Skills Required:
·Completes all mandatory competency testing.
