Covenant Case Management Services is seeking a Nutritionist.
Brief Job Description: Provide expertise, training and technical assistance to assist family members, support staff and other natural supports in assisting participants with developmental disabilities who have long term intervention needs. Under this model, family members and other paid/unpaid caregivers are trained by a Nutritionist (with I/DD experience) on specific nutritional needs of the individual, consistent with the Individual Support Plan. This service will also be utilized to allow the LIP (Licensed Independent Professional) to be an integral part of the Individual Support Team to participate in team meetings and provide additional nutritional consultation and support for participants whose medical and behavioral/psychiatric needs are extreme or complex. Participate in team meetings; and/or, tele-consultation through use of two-way, real time-interactive audio and video between places of lesser and greater clinical expertise to provide nutrition service when distance separates the care from the participant. In-person support may be required depending upon the location of the assigned member.
Qualifications/Requirements: Experience with Intellectual/Developmental Disabilities (I/DD), Autism Disorder, and/or Traumatic Brain Injury (TBI) Familiarity with NC Innovations Waiver (preferred, but not required) Complete criminal background check (no health and safety risk to individuals): Not listed in the North Carolina Health Care Abuse Registry. Qualified in the customized need of the participants as described in the Individual Support Plan. Hold appropriate education and North Carolina State License or Board Certification required for Specialty.
