The MDS/Care Plan Coordinator performs nursing assessments of residents and documentation review and analysis as needed for completion of resident’s MDS, CAAs and Care Plan in accordance with standards of nursing practices, regulations, and RAI Manual. The MDS/Care Plan Coordinator may lead or direct licensed and non-professional facility staff in the assessment and completion of MDS, CAA and Care Plan process.
Requirements:High-School Diploma or equivalency required. Associates, Bachelor’s degree or university program preferred.
2-4 years related experience and/or training or equivalent combination to education and experience required.
Experience as a MDS/Care Plan Coordinator preferred.
Long term care experience preferred.
LPN or RN license
