HOME DELIVERED MEAL ASSESSORTemporary ClericalDivision of Aging, Disability & Behavioral Health Services The Home Delivered Meal Assessor is responsible for evaluating and ensuring the appropriateness of meals delivered to individuals. Client interaction and assessment of needs will be a focal point in this position. The assessor will also facilitate quarterly nutrition advisory council meetings. This temporary position will report to the Manager of Aging and Disability Services and will work 20-30 hours per week. Kenosha County is committed to Equity, Diversity and Inclusion and is anEqual Opportunity Employer. 2024 Hiring Range: $20.31 - $23.08 per hour Position Summary and Job DutiesInterview meal program participants and complete in-home assessmentsCommunicate assessment outcomes to Home Delivered Meal CoordinatorMake referrals to community programs, services, and resources as neededParticipate in meetings and annual trainingsPractice competent motivational interviewing techniquesSupport and enhance a trauma-informed care environmentComplete annual compliance training and adhere to Human Service compliance policies and proceduresAdhere to and promote safety as a priority in the workplacePerform data entry of in-home assessmentsFacilitate Nutrition Advisory Council meetingsPerform other duties as required or assigned Success Factors Knowledge of:Community resources for elders and persons with disabilitiesWisconsin statutes, specifically Chapters 46, 54, and 55Skill in:Use of Microsoft Office Suite, including Word, Outlook, and ExcelOral communicationWritten communication including the ability to craft correspondence and proper documentation with minimal grammatical and syntax errorsOrganization and time managementMotivational interviewingBasic internet usageAbility to:Travel approximately 75% of time for home visits in Kenosha CountyTake accurate notes and details regarding assessments and family contactEffectively collaborate and communicate with external agenciesWork independently, multi-task, prioritize, and meet time requirementsRemain professional and courteous at all timesMaintain confidential informationWork respectfully in a diverse and inclusive environment Job Requirements, Education, Training and ExperienceRequired Education and ExperienceHigh school diploma or GED equivalentOne to two years of work experience in Human Services fieldSixty credits post-secondary education, especially in a Human Services or Social Services related field of studyPreferred Education and ExperienceAssociate degree in Human Services or Social Services related field of studyExperience with elders and persons with disabilitiesExperience with individuals of varying socio-economic backgroundsOr any combination of education, training, and experience which provides the required knowledge, skills, and abilitiesOther Job RequirementsValid driver’s license and acceptable driving recordAccess to a personal vehicle for use in the performance of the job Physical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of the job.
