Company Overview: Country Manor Health Care & Rehab Center is a leading provider of skilled nursing and rehabilitation services dedicated to enriching the quality of life for our residents. Our commitment to excellence extends to every aspect of our operation, including the coordination and implementation of the Resident Assessment Instrument Process (MDS). We are currently seeking a skilled and experienced Registered Nurse to join our team as a Skilled Nurse Assessment Coordinator.
Location: Sartell, Minnesota
Position Overview: The Skilled Nurse Assessment Coordinator (SNAC) is responsible for the coordination and implementation of the Resident Assessment Instrument Process (MDS) in accordance with state and federal regulations. The SNAC will communicate and coordinate health information to skilled payers, ensuring accurate and optimal reimbursement for resident care. The ideal candidate will have current RN licensure in the State of Minnesota, with two or more years of RN experience, along with a strong understanding of the MN Case Mix System, MDS process, and general resident assessment requirements. Country Manor is willing to provide training for qualified candidates.
