Case Coordinator Job Description

The Case Coordinator coordinates post-release and community based behavioral health and supportive services for participants enrolled in a telebehavioral health program with Comprehensive Recovery Services. In collaboration with the client, the Case Coordinator will develop a proactive treatment plan with the client before their release from the Bristol County Jail and House of Correction. The Case Coordinator will provide follow-up support to clients in the community to assist them in overcoming care obstacles and promote favorable client outcomes.

Key Responsibilities

  • Client Assessment: Conduct assessments to identify the specific needs and challenges of individuals seeking services.
  • Information and Referral: Educate and enroll participants in the Telebehavioral Health Care (TBH) program.
  • Individualized Planning: Develop post-release personalized case management plans for clients transitioning from Bristol County Jail and House of Correction into the community.
  • Coordination and Collaboration: Coordinate client treatment plan with CRS. Collaborate with other social service agencies, community organizations, and healthcare providers to enhance the network of available resources. Attend meetings with BCSO and CRS to promote effective transition and care coordination.
  • Education: Provide information and assistance around the technology needed to use telehealth services (telehealth training and support will be provided for all staff). Provide clients with information on available social services, resources, and programs and guide them through the process of accessing these services.
  • Post Planning Support: Travel throughout the Bristol County area to meet clients in public establishments to enhance continuity of client care through ongoing community outreach connections.
  • Advocacy: Advocate on behalf of clients to ensure they receive appropriate services and support. Work with service providers and community organizations to address barriers to access.
  • Documentation: Maintain accurate and up-to-date records of client interactions, assessments, and service plans. Record, track, and maintain client data as required.
  • Other duties and responsibilities as assigned by the Director/Supervisor.

Qualifications

  • Bachelor's or Master's degree in social work, psychology, counseling, or substance abuse credential of CADC or LADC preferred.
  • Knowledge of local social services, community resources, and support programs in the Bristol County area.
  • Strong communication and interpersonal skills.
  • Empathy and a non-judgmental approach when working with diverse populations.
  • Problem-solving skills and the ability to think critically in complex situations.
  • Familiarity with relevant laws, regulations, and ethical standards in social services.
  • Cultural competency and the ability to work with individuals from diverse backgrounds.
  • Reliable transportation, valid MA driver’s license, auto insurance, and registration.

Additional Skills

  • Proficiency in using technology and databases to access and manage information.
  • Crisis intervention skills.
  • Lived experience around mental health, criminal justice or substance use issues preferred but not required.
  • Ability to work collaboratively in a team and independently when necessary.
  • Bilingual, English-Spanish, strongly preferred.

Location

  • Bristol County, MA - Travel Required throughout Bristol County and to the Bristol County Jail and House of Correction in Dartmouth, MA

Schedule

  • Part-time, 24 hours a week
  • Flexible schedule

Salary range

  • $32-$36/ hour
  • Mileage reimbursement
