Job Summary
Provide leadership to the direction, planning and execution of all aspects of quality, care management and population health within Lakeland Care Network, connecting organizational clinical outcomes and corresponding data analytics with performance improvement activities to improve and/or maintain health system quality with payers, external agency requirements and other quality initiatives. Develop partnerships across all levels of the care continuum, including physicians, the health system, payers and community organizations. Lead a performance improvement and care management team to administer large scale transformation of healthcare in the community. Ensure consistent, high quality service and culture while maintaining financial integrity and accomplishing the objectives and strategic initiatives of LCN and its Board.
Essential Functions
  • Ensure the Lakeland Care Network physician led strategic goals and objectives are successfully accomplished, using influence as the main method, by developing trusting and collaborative relationships with all Lakeland Care medical practices (employed and independent), Lakeland Regional Health System and Affiliated Services, community organizations and services, Michigan Federation Care Network (Clinically Integrated Network), Payer Entities and Vendors.
  • Develop, direct and execute LCN and LRHS strategic plans as they pertain to quality, care management and population health initiatives across the 3 counties of Berrien, Cass and Van Buren. Initiate new objectives which align to LCNs mission and vision. Establish data quality standards and workflows with various reporting agencies to ensure standards are met. Enforces quality data collection techniques with various reporting registries and agencies. Ensures proper transmittal of data to health system reporting registries, agencies and systems.
  • Lead large scale change management initiatives across the health system and community with best practice methodology as a framework and an understanding of re-engineering current systems and processes.
  • Serve as a leader in performance improvement and best practices initiatives, encouraging 'best of the best' mindset while championing the development and implementation of best practices and programs. Committed to the continuous improvement of the patient experience which includes patient safety, quality of care and excellent service.
  • Responsible for hiring, leading and developing a quality and care management team. Effectively champion LCN’s mission and values and empowers and ensures that all associates achieve the designated organizational goals. Build and maintain a high performance team culture through effective performance management, development, communication and coaching of team members resulting in a focused on developing collaborative and trusting relationships across the continuum of care and broader community to advance population health while being positive ambassadors communicating the benefits and values offered by LCN. Responsible for employee evaluation and progress toward role objectives. Identifies areas of opportunity and implements development plans to achieve successful results or indicates alternatives.
  • Oversee and direct quality and care management staff to ensure continual compliance with quality program(s) such as payer contracts, accreditation bodies and regulatory standards. Directs analysts in measurement and tracking of program effectiveness.
  • Collaborate with Data Analytics and IT to determine data needs, security requirements and analytic resources for LCN. Promote the adoption of system wide tools that enhance efficiency, convenience, access and the delivery of safe patient care. Collaborates with Information Security to formulate, implement and enforce data policies and procedures. Collaborate with Medical Staff to develop, evaluate and report provider clinical quality goals and outcomes as part of Ongoing Professional Practice Evaluation (OPPE), Clinical Integration and other physician quality/performance outcome initiatives.
  • Prepare and submit annual capital and operational budgets, anticipating future needs based on projected volume, workload, depreciation, environmental needs, programs and other factors. Monitor operations and budget in order to identify various variances and to research and report on causes.
Qualifications

Required

  • Bachelor's Degree in Business Management or a related field.
  • 5 years of relevant experience of progressive experience in management and/or quality management.

Preferred

  • Master's Degree in Healthcare or related field.
  • Knowledge of healthcare management, quality improvement and data analytics methodologies
  • CRT-Project Management Professional (PMP) - PMI Project Management Institute
