Job Summary

Coordinates educational programs and activities designed to enhance clinical drug knowledge of staff pharmacists, physicians, nurses, other care providers. Works with staff pharmacist to increase their participation in drug monitoring, pharmacokinetics and cost containment activities. Provides pharmaceutical care to patients in assigned area of practice, implement cost containment programs, provide drug information, education and clinical services which enhance the effectiveness of drug therapy and minimize patient risk. Works with multiple medication automation systems.

Clinical Pharmacy Specialists within Primary Care

  • Work closely with providers under a Collaborative Practice Agreement (CPA) to evaluate and optimize medication regimens for mutual patients.
  • Provide guideline and evidence based Chronic Disease and Medication Therapy Management directly to patients within a clinic setting.
  • Assist patients with cost, adherence, and lifestyle barriers related to their medications.
  • Work closely with clinic staff to inform them of guideline and medication updates.
  • Collaborate with other care team members to develop care plans.
  • Navigate EPIC and utilizes features within the EMR daily.

Essential Job Functions

  • Provide chronic disease state management and/or medication therapy management directly to patients within the clinic setting. Pharmacists will work under a collaborative practice agreement within the clinic they are embedded to provide medication management directly to their patients.
  • Build relationships with providers and staff by living our values to promote trust and gain referrals within the clinic.
  • Follow Corewell Health’s standard work, policies and procedures to gather and interpret patient vitals and labs during visits. Locate and use clinical data within the EMR to assist with developing future treatment plans.
  • Participate in team-based approaches to help determine the best treatment plan for the patient. In addition, communicate within the clinic care team to determine which patients would benefit the most from Pharmacy services.
  • Collaborate with providers and office staff on a regular basis to facilitate educational updates on guidelines, new medications or therapies, and best practices. Keep the clinic updated on any medication related changes.
  • Assist clinic in monitoring and improving upon system Quality Measures including, but not limited to, MTM, A1c, Hypertension, Adherence, and Readmissions.
  • Help patients and clinic staff navigate formularies and other programs to assist in lowering the cost of care for patients and the system.
  • Engage in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities. Complete required continuous training and education, including department and Spectrum Health specific requirements on a timely basis.
  • Precept pharmacy residents and students independently as assigned.
  • Evaluates patients and orders, and takes a lead role in the ordering and interpretation of drug serum levels, using pharmacokinetic principles. Provides physicians with dosage regimen recommendations to enhance therapeutic outcomes, reduce patient’s risk, and reduce the cost of drug therapy.
  • Provides drug information and educational services to hospital health care professionals, patients and families and assists staff pharmacists in responding to drug information requests. Serves as an information resource and clinical practice role model for staff pharmacists.
  • Coordinates and implements departmental drug utilization review efforts and other process improvement activities as assigned.
  • Ensures that processes and services are continuously monitored for quality, cost effectiveness, and efficiency. Engages in process and quality improvement activities. Makes and implements recommendations to improve operational efficiency and to implement new services for areas of responsibility.
  • Facilitates applicable Pharmacy Committee activities in area of specialty and participates on other committees / task forces as member, consultant and/or chair as assigned.
  • Engages in continuous study of the entire professional field to maintain the professional competence, knowledge, and skills necessary for satisfactory performance of all assigned responsibilities. Completes required continuous training and education, including department and Spectrum Health specific requirements on a timely basis.
  • Coordinates the development, editing, and dissemination of departmental publications for physicians and hospital staff.
  • Precepts pharmacy residents and students as assigned.
Qualifications
  • Required Bachelor's Degree Science of Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy
  • 2 years of relevant experience pharmacy experience or an American Society of Health-System Pharmacists (ASHP) Pharmacy Practice Residency Required
  • 2 years of relevant experience hospital pharmacy experience or an American Society of Health-System Pharmacists (ASHP) Pharmacy Practice Residency Preferred
  • ASHP accredited postgraduate year 2 (PGY2) residency in area of specialty Preferred
  • LIC-Clinical Pharmacist - STATE_MI State of Michigan 90 Days required
  • LIC-Pharmacist Controlled Substance - STATE_MI State of Michigan 90 Days required
  • CRT-Basic Life Support (BLS) - AHA American Heart Association Ambulatory Sites 90 Days required
  • CRT-Pediatric Adv Life Support (PALS) - AHA American Heart Association 180 Days required Or
  • CRT-Adv Cardiovascular Life Support (ACLS) - AHA American Heart Association 180 Days required
  • CRT-Board Cert Pharmacotherapy Specialist (BC-PS) - ASHP American Society of Health-Systems Pharmacists Upon Hire preferred Or
  • CRT-Board Cert Pharmacotherapy Specialist (BC-PS) - BPS Board of Pharmacy Specialties Upon Hire preferred
  • Ideal candidate would be a Pharmacist possessing PharmD Degree as well as experience or Certification in Ambulatory Care
  • Ideal candidate will possess a PGY1 Residency or PGY2 Residency in Ambulatory Care and Direct Patient Care experience
  • BCACP Board Certification would be highly preferred
  • We would welcome someone with a Community Pharmacy background if they demonstrate the ability to engage with Physicians, Insurance Payers, and Patients to optimize Medications
  • Hours are Monday to Friday
  • No Weekends
  • 8 a.m. to 4:30 p.m.
