Activity Director Job Description-TUSTIN

MONDAY THRU FRIDAY 8AM-430 PM

Qualifications

Passion and commitment to the mission and work of the organization

Self-driven and ability to operate both independently and with the flexibility to be part of a team

One-year experience in a social, recreational, or educational program with an emphasis on seniors or disabled adults in a health care setting, mental health setting .

Physically capable of both performing the required duties and of assisting frail elderly and disabled adults, as necessary

Possess strong computer skills and functional knowledge of the Microsoft Office Suite (Word, Excel)

Copies of current and applicable degree(s), license(s), certification(s), First Aid, and Healthcare Provider CPR have been provided to the Center

Per public health order (until CDC updates 4/4/23) the Covid-19 vaccination is mandatory for employed healthcare workers

• Preferably one or more years working with the elderly..
• Must be computer-savvy and comfortable working with an industry software.

Responsibilities

Aspires to provide Person-Centered care theories and practice as required by CDA/CBAS with the understanding that a care plan shall reflect the individual's strengths and preferences, reflect clinical and support needs as identified through an assessment of functional need, and include individually identified goals and desired outcomes

Adheres to all licensing, regulation, and Center policies and procedure

Conducts an initial assessment of each participant, and subsequent reassessments of Person-Center activity services provider

Develop a planned schedule of social and other enriching individual or group activities

Create a monthly activity calendar

Manage the volunteer/ intern responsibilities, general orientation, and volunteer file management process, communication and general follow-up pertaining to volunteer interest.
