Care Coordinator

As a Care Coordinator in Intensive Behavioral Health Services, you will:

  • Provide supports to the children and families served as an adjunct to their clinical care within the identified cluster area.
  • Develop rapport and relationships with clinical staff, working with them to ensure clients receive the highest quality of care.
  • Meet regularly with parents and clinicians to assess the progress being made toward the goals of the treatment plan.
  • Assess the child/youth's overall wellbeing, provide supports to address the identified physical and behavioral health needs and engage/re-engage as needed during the course of services.
  • Process and coordinate all referrals into the program, which includes ongoing services for our clients as well as the discharge process.
  • Assess and evaluate related social determinants of health to be integrated into a robust, person first service plan for each family.
  • Meet regularly with staff to receive updates on the progress of cases.
  • Mediate difficult situations with school and community systems, advocating for the clients and families as needed.
  • Successfully deliver services in coordination with multisystem involvement and programmatic requirements.
  • Provide administrative supervision, monitoring, and case management for each case until the client is discharged.
  • Oversee caseload list per school in order to ensure that all authorizations are current on a weekly basis.
  • Complete CBH packets and interagency meetings prior to original authorization, for every authorization cycle or as needed based on client's treatment requirements.
  • Ensure all new referrals are processed or listed as non-active referrals within 30 days of receipt.
  • Coordinate recommendations made by evaluators by following up on appointments and referrals, completing home visits, and arranging for transportation for clients as needed.

Qualifications:

  • Bachelor's degree in a Human Services field
  • Life experience in navigating the behavioral health and other service systems preferred.
  • Experience providing care coordination in a health care and/or human service field for individuals requiring access and linkage to needed resources.
  • Understanding of and experience in navigating social service systems, such as physical health, education, employment, housing, insurance etc.
  • Experience utilizing an electronic health record (EHR) for documentation of service delivery.
  • Demonstrated understanding of cultural competence, with the ability to develop supportive and trusting relationship with individuals needing assistance.
