Monroe County Department of Human Services, Child and Family Services (CFS)
Rochester, NY On-Site
FULL-TIME Nonexempt : 40 hours per week

Coordinated Care Services Inc (CCSI) seeks a talented Behavioral Health Specialist to contribute their expertise in providing essential business services to our provider organizations and customers. The Child and Family Services Behavioral Health Specialist (CPS) provides consultation and linkage to youth and families, identified by Child Protective Services, who present with an array of behavioral health needs. The primary purpose of the Behavioral Health Specialist CPS is to ensure behavioral health and developmental needs are considered, actively identified and addressed for all youth involved with DHS Child and Family Services, in order to prevent further involvement into the DHS system, while promoting better outcomes and hope for youth and families. The Behavioral Health Specialist will work closely with FACT, Child Protective Services and community providers to stabilize youth and family, avoiding placement into DHS Foster Care.

CCSI is partnering with Monroe County Department of Human Services to hire a Behavioral Health Specialist. This position is employed by CCSI and working on behalf of Monroe County

In this role, you will be [add verb tense or additional action words to give context to duties]

  • Participate in consultation groups or individual meetings in which creative collaborative planning is needed to keep youth safely in the home.
  • Attend meetings with CPS and other service providers as requested by CPS supervisors, with goals of assessing complex challenges and contributing to stabilization planning for youth and family.
  • Utilize trauma-informed lens to inform service planning and decisions.
  • Develop expertise of community resources and interventions to support youth at risk of out-of-home placement.
  • Successfully navigate cross system youth and families and leverages appropriate responses.
  • Utilize Hope Science with youth and families, fully integrating Hope into goal-setting and all aspects of collaboration and support.
  • Collaborate with CPEP and CPS to work with families to develop plans that support youth leaving CPEP to return home.
  • Participate in CPS, FACT and OMH Children’s Priority team meetings.
  • Hold provider meetings in order to ensure all services for youth and families are working together to support Monroe County System of Care.
  • Maintain Salesforce and FFF data to meet BHS CPS metric.
  • Other duties as assigned.

Requirements

Education and Experience

  • MSW or other human services Master’s preferred, plus three years’ experience in children’s mental health, or equivalent education and work experience (see CCSI's equivalency chart).

Additional Requirements

  • Satisfactory completion of all required background screenings.
  • Valid Class D New York State Driver License and access to motor vehicle to meet transportation requirements of position. 
  • Ability to work flexible hours including evenings and weekends, as needed.
  • Respect the confidential nature of all CCSI and CCSI customer information you are exposed to in the course of your work performance. Staff working directly with protected health information/electronically stored protected health information, personally identifiable information, and other patient/consumer/customer data, will abide by the specific procedures and policies outlined by CCSI’s Code of Conduct and CCSI’s Privacy and Security Policies, New York State regulations and Federal regulations.
