Duties Include:

• Assessing strengths and needs
• Advocating for individuals
• Facilitating meetings
• Writing Individual Plans
• Linking to resources
• Making referrals
• Securing funding
• Monitoring services provided
• Documenting activities

Education and experience requirements:

• Bachelor’s degree from an accredited college or university
• Valid driver's license & proof of insurance. Access to clean, reliable transportation
• Must be willing & able to visit clients in their own homes
• Post offer drug screen and back ground check
