Role Overview

The Diabetes Education Program Coordinator is responsible for providing diabetes and nutrition counseling for patients in an outpatient setting. They coordinate the activities of the Diabetes Management Programby assisting in the development and maintenance of procedures, policies, standards of practice, and quality improvement initiatives for the program while ensuring compliance with American Diabetic Association's Program Recognition guidelines.

In conjunction with the department Medical Director and Practice Manager the Outpatient Diabetes Program Coordinator:

  • Works to ensure the Diabetes Management Program is in compliance with updated Medicare and insurance coverage guidelines
  • Informs program staff of changes and keeps all relevant materials updated in Medicare/Insurance Manual
  • Provides comprehensive education, assists in development of comprehensive treatment plans and makes treatment recommendations for each diabetes population including Type 1, Type 2, Pre-diabetes & Gestational Diabetes
  • Develops and implements age appropriate and individualized nutrition care plans based on assessment of patient's nutritional status, educational needs, and stated goals
  • Counsels and educates patients and their families
  • Collaborates with the interdisciplinary health care team in the treatment of individuals and groups.
  • Performs other duties as required.
Qualifications

Qualifications:

  • Bachelor's degree in Nursing, Public Health, Nutrition Sciences, or related field required; Master's degree preferred
  • Certification by the National Certification Board for Diabetes Educators to be completed within six months of employment
  • One (1) to three (3) years of experience in a healthcare environment required
  • Demonstrated oral and written communication skills required
  • Demonstrated project leadership, both independently and as part of a team required
  • Supervisory experience preferred
