JOB DESCRIPTION

Job Title: Child and Family Services Navigator

Department: Child and Family Services Department

Reports To: Operations Manager

Supervises: None

FLSA Status: Non-Exempt

Pay Grade: N4

Job Type: Regular, Full-Time

General Functions:

The Child and Family Services (CFS) Navigator provides navigation support and linkages to essential resources to all participants who contact and/or are enrolled in the CFS programs, as appropriate for the service model of each program. The CFS Navigator provides specific support for the referral and intake process for department classes. In addition, this role is responsible for supporting cross-program cultural activities, special events, and participants who are enrolled in multiple CFS programs. The CFS Navigator provides essential support to cross-program coordination related to incoming referrals, satisfaction surveys, exit surveys, post-enrollment follow-up with participants, program reporting, and the departmental Continuous Quality Improvement/Continuous Quality Management (CQI/CQM) team.

Duties and Responsibilities, including but not limited to:

  • Provide support, information, crisis de-escalation, and referrals to walk-in participants and callers contacting the front desk in need of immediate assistance. 
  • Assist the general public with the referral process to specific CFS programs by providing accurate program information, pre-screening the participant for eligibility, and completing appropriate referral forms.
  • Navigation across CFS Programs: 
    1. Identify essential resources to meet the family’s needs and coordinate warm hand-off to services, including providing resource information, scheduling in person meetings, assisting participants with referrals and applications, and providing transportation as appropriate.
    2. Assess the outcomes and effectiveness of referrals made, and adjust referral sources to families as needed.
    3. Establish positive relationships with community agencies to enhance participants’ access to essential resources. 
    4. Provide updates to program staff on navigation services provided to their enrolled participants.
    5. Document referrals and outcomes in the electronic database. 
  • Referrals and Intake Process for Department Classes:
    1. Review, process, and document incoming referrals, and track referral follow-up timelines to meet program deadlines. 
    2. Assist participants, other agencies, and/or staff with the Office of Children’s Services with completing the correct referral form. 
    3. Complete follow-up with participants, and coordinate the intake process, including scheduling and preparation for intake.
    4. Conduct regularly scheduled group intakes and also individual intakes in the event of special circumstances.
    5. Ensure all paper intake forms are completed accurately, signed, and uploaded into the electronic database.
    6. Complete all intake and enrollment data entry in the electronic database. 
    7. Assist Program Manager with program reporting related to referral and intake data. 
  • Cross-Program Cultural Activities and Special Events: 
    1. Support staff with preparation prior to the event and clean-up following the event.
    2. Participate in cultural activities when appropriate and as requested.
    3. Document participant attendance in cross-program events in the electronic database as assigned.
    4. Serve on the organizational Activities committee as a representative of the department, and participate in the planning and preparation for special CITC events.
    5. Create and distribute fliers and other materials for upcoming participant activities.
  • Cross-Program Coordination:
    1. Coordinate with the managers’ team to gather information and prepare weekly programming updates with CITC’s Communications team. 
    2. Monitor, route and process incoming referrals in the electronic database.
    3. Identify participants in the database who are multiply involved, inform relevant programs, and provide brief summary updates on participant cross-program activities on a monthly basis.
    4. Distribute and document results of participant satisfaction surveys. 
    5. Administer and document results of program exit surveys.
    6. Conduct post-discharge interviews and follow-up with participants.
    7. Participate in departmental CQM/CQI activities to improve the quality, effectiveness and efficiency of program services as relevant for this role. 
    8. Provide general support for departmental participant services data entry and reports as assigned. 
  • Abide by both CITC and CFS policies and procedures.
  • Participate in individual administrative supervision, team planning, and department meetings.
  • Maintain professional and technical knowledge by attending trainings as directed; reviewing relevant publications; and establishing professional networks to exchange knowledge.
  • Maintain participant confidence and protect operations by keeping information confidential.
  • Contribute to programming effectiveness by accomplishing related results as needed.
  • Perform all related duties as needed and assigned.

Job Specifications:

  • Flexibility in the work schedule is required to meet the participants' needs.
  • Regular attendance is required to maintain continuity of services.
  • Regular travel is required to participate in cultural activities.
  • General knowledge and understanding of the social, health, educational, training, and cultural needs of Alaska Native and American Indian community.

Minimum Core Competencies: CITC Values, Respectful Leadership, Professionalism, Emotional Intelligence, Problem Solving/Critical Thinking, Communication Skills.

Minimum Qualifications:

  • Bachelor of Arts or Sciences in Social Work, Psychology, Counseling, or related field. Relevant experience may substitute for the educational requirement on a year-for-year basis.
  • Two years of experience working with adults, children or families in a human services or related field. 
  • Continued employment is contingent upon receipt of satisfactory reports from a state and federal background check.
  • Valid Alaska driver’s license and insurable under CITC’s automotive insurance, which requires a driver to be at least 21 years of age and have had a driver’s license for at least three years. 

Preferred Qualifications:

  • Experience in interviewing and supporting participants with ongoing adverse experiences and trauma preferred.

Physical Requirements:

  • Lifting, pushing and pulling up to 50 lbs. intermittently, standing for a long period of time,stooping and bending will be required. This position will involve exposure to weather.
