As a Consulate Health Care Interim-MDS Coordinator, you are entrusted with the responsibility of caring for our residents, families, coworkers, visitors and all others; as well as demonstrating in all interactions, Consulate Health Care’s five core values of Compassion, Honesty, Integrity, Respect and Passion.
The primary purpose of your job position is to coordinate delivery of services to managed care and Medicare residents in collaboration with the facility’s team members. The Interim-MDS Coordinator will assist the Director of Clinical Services with ensuring that all documentation in the facility meets all federal and state, certification guidelines. The MDS Nurse will coordinate the entire RAI process assuring the accuracy, timeliness, and completeness of the MDS, RAPS, and Interdisciplinary Care Plan. The MDS Nurse monitors and documents the cost effectiveness of treatment provided, facilitates and coordinates the admission and discharge process, serves as the resident and family advocate and acts as a liaison to insurance and medical management professionals. You are entrusted to provide innovative, responsible healthcare with the creation and implementation of new ideas and concepts that continually improve systems and processes to achieve superior results. Job Functions : As Interim-MDS Coordinator, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties. Responsible for providing direct resident care and accordance with established plans. No supervisory function. This job description does not list all the duties of the job.
