SUMMARY:
Coordinate all aspects of care for people to ensure active treatment is provided on an ongoing basis in the least restrictive environment, as per the OPWDD.
DUTIES:
- Coordinate individuals’ services with Care Manager and other outside service providers.
- Develop and implement goal programming and activity schedules.
- Ensure individuals’ personal environment is clean, safe, and well-maintained.
- Ensure individuals’ finances and personal property are appropriately managed and protected.
- Obtain/maintain agency van certification and transport as needed.
- Maintain communication with family/correspondent
REQUIREMENTS:
- AMAP certified (or ability to obtain certification w/in 3 months of hire)
- Agency driver authorized (or ability to obtain authorization w/in 3 months of hire
- Valid NYS Driver's License
Job Type: Full-time
Pay: $50,000.00 - $55,000.00 per year
