Constellation Health Services is Hiring a Registered Nurse On Call 7 on 7 off Near Augusta, ME

Registered Nurse (RN) On Call 7 days on 7 days off!

Job Summary:

Constellation Hospice is seeking an RN On Call who will be responsible for ensuring that patient care is coordinated and managed appropriately during the hours that the office is closed. A common schedule will be from 5pm to 8:30am with 7 days on and 7 days off. The On-Call RN is responsible for ensuring that care and services are delivered appropriately. The RN will provide emergency visits and pronouncements during scheduled on call time throughout our service area. Our service area includes but is not limited to Augusta, Bangor, Kennebunk and Rockland, Maine.

Responsibilities:

  • Deliver compassionate and comprehensive nursing care to patients facing end-of-life challenges.
  • Conduct thorough assessments to identify patient needs and develop individualized care plans.
  • Manage medications and treatments as prescribed by the physician.
  • Provide emotional support and guidance to patients and their families.
  • Educate patients and families about hospice care and end-of-life issues.
  • Collaborate effectively with interdisciplinary team members to ensure coordinated care.
  • Maintain accurate and up-to-date patient records.
  • Participate in ongoing education and training opportunities to stay abreast of the latest advancements in hospice care.

Qualifications:

  • Current and active RN license in the state of employment.
  • Bachelor’s degree in nursing (BSN) preferred.
  • Minimum of 1 year of experience in hospice care or end-of-life care setting.
  • In-depth understanding of end-of-life care principles and practices.
  • Exceptional communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Compassionate and empathetic approach to patient care.
