Per Diem - Physical Therapist (PT)

Job Description:

We are looking for committed Physical Therapists who can travel throughout an assigned territory and act as the patient’s partner throughout the trip of restoring motion and ensuring that the patient will function at their personal best. You will maximize the quality of life and movement potential.

Responsibilities:

• Participate in the development of the individualized plan of care.
• Provide exceptional Physical Therapy - PT to patients by assessing and understanding care plan.
• Evaluate patient's functional status and home environment for hazards or barriers to more independent living. Identify equipment needs.
• Maintains and processes all documentation in compliance with agency standards utilizing the electronic medical record keeping system.
• Submits all clinical documentation, time records and other information in the required time frame.
• Educate patients and caregivers on disease process, medications, plan of care and individualized treatment plan.
• Supports company mission by providing the highest quality of home care.

Qualifications:

• Licensed Physical Therapist-PT in the state of New York with at least 1 year of clinical experience; Home Care preferred
• COVID Vaccinated
• Proven work experience as a physical therapist
• Graduate degree or greater in physical therapy
• Valid license to practice physical therapy
• Oasis experience strongly desired
• Have valid driver’s license, reliable transportation and willing to travel
• Willing to take advantage of our excellent training system comprised of in house and field orientation
• Have great communication skills, excellent organizational, and time management skills
• Problem-solving skills
• Have great interpersonal skills
• Excellent computer skills
• Ability to work independently with minimal supervision.
