Connecticut Dental Health Partnership is Hiring an Oral Health Navigator - HUSKY Health Dental Plan Near Farmington, CT

Overall Responsibility: Conduct oral health care navigation for HUSKY Health (Medicaid) members to link members to oral health care services. The goal of the Oral Health Navigator is to reduce member’s internal and external barriers to accessing dental care, support obtaining necessary treatment for acute needs, and improve member’s oral health literacy and oral health behaviors.

Report to: Oral Health Navigation and Community Engagement Manager

Key Responsibilities/Scope of Work:

1. Execute Oral Health Navigation functions to assigned members, including:

  • Engaging, contacting, and fostering a trusting relationship with members, to meet members “where they are at”:
  • Assessing member’s oral health, social determinants, and barriers to care.
  • Identifying and collaborating with members to achieve goals including dental provider's oral health treatment plan and home care goals.
  • Assisting members in locating participating dental providers.
  • Assisting members in securing appointments including transportation and other supports.
  • Assisting members in arranging translation supports as necessary.
  • Helping members understand the dental benefits available under their applicable program.
  • Assisting members in overcoming other barriers that keep them from receiving needed oral health care including dental trauma/phobia, social, medical, and behavioral health barriers to care.
  • Provide in-person support as deemed appropriate to provide oral health navigation services.
  • Assessing member’s comprehension or oral health instructions from dental providers and support member to comply with home care instructions.
  • Work collaboratively with referral sources including health systems, state agencies, community organizations and partners in co-managing members to meet both oral health, medical, and behavioral health needs.
  • Referring members to social service support to reduce external barriers to care.
  • Identifying and address internal barriers- dental anxiety/phobia, beliefs and attitudes about oral health practices, lack of knowledge about oral health and prevention, and low prioritization of oral health.
  • Documenting actions in care coordination platform according to CTDHP standards.

2. Participate in case consults, escalations, and case conferences to identify new interventions, receive guidance and suggested actions working with the member.

3. Foster a spirit of teamwork and unity among the team that focuses on continuous feedback, solution development, and ongoing communication to improve processes, performance, and outcomes.

4. Commitment to continuous growth and professional development, including becoming a Certified Community Health Worker during your tenure at CTDHP.

Skills and Attributes:

  • Commitment to and demonstration of cultural and linguistic competency that empowers our colleagues, teams, and especially our members.
  • Understanding of social determinants/drivers of health and its connection to oral health equity.
  • Orientation to and the practice of member-centered care, non-judgement, empathy and sensitivity to cultural beliefs relative to health-related decision making, including oral health.
  • Understanding of care coordination functions and principles.
  • Receptivity to delegation of tasks and practicing “managing up” to Manager.

  • Ability to meet, and drive to exceed, expectations and standards of case load and regional outreach efforts.
  • Diplomatic and professional demeanor in external settings recognizing representation of both CTDHP and the Department of Social Services
  • Proficiency with Microsoft Office
  • Ability to process information through computer systems
  • Ability to adjust priorities and manage time wisely in a fast-paced environment
  • Ability to maintain records and documentation and understand the importance of documentation in care coordination.
  • Ability to communicate in a clear, concise, understandable manner, and listen attentively to others, understand material, and provide instructions to others.

Experience:

  • Bachelor’s degree in social work, human services, community/public health and social services strongly preferred.
  • Experience in care coordination, care management, and community health care navigation including health system, Medicaid Waiver programs, and behavioral health navigation work preferred.
  • Experience with working with populations that face barriers to accessing health care delivery and social supports a plus too.
  • Bilingual skillset highly welcomed and prioritized.
