Description
We are seeking an experienced Nurse, Social Worker, or Care Manager to help support our multidisciplinary team. This role is part time, Per Diem, based in Bridgeport, Connecticut working with a group of 12-15 older women who are part of a religious community. Most residents are retired with varying levels of care needs. They live in individual units within a large apartment building and gather as desired in a common space for meals, socializing, and scheduled meetings and activities.
Essential Functions:
• Facilitates the implementation of resident’s individualized life plan.
• Determine individualized strategy and support needs which may include ongoing active support.
• Advocates for residents needs during medical appointments and clinical settings, as necessary.
• Arranges transportation for and accompanies residents to medical appointments and other meetings, outings, or appointments, as needed.
• Participates in after hours on-call rotation.
• Acts as part of a multidisciplinary team of professionals supporting residents.
• Meets regularly with Team.
Requirements
Education
· Bachelor’s degree in administration nursing, social work, public health, psychology, counseling, gerontology, or related field required.
Experience:
· Must have a minimum of 5 to10 years of experience in health care or social services or related field plus experience working with older adults.
· Knowledge and understanding of social, health, and economic factors that contribute to health and wellbeing outcomes; knowledge and skill in interviewing and assessment (social and health) techniques.
· Knowledge of community resources available to individuals; an ability to mobilize resources into a coordinated and comprehensive plan of care.
· Strong written, verbal and computer skills required.
· Ability to maintain confidentiality of resident, company, and staff information.
